INSTRUCTIONS TO THE APPLICANT

Detailed below are the contents of the Supporting Documentation Package which must be
submitted with your request for funding for FedNor's Youth Internship Program-Private Sector
Component. This normally provides sufficient information to permit an assessment and decision
on your application. The Officer assigned to your project may request additional information or
clarification of the data provided. All information provided will be kept strictly confidential.

Please complete and sign the accompanying FedNor request for funding form and return it with
the Supporting Documentation Package.

The following checklist has been provided for your convenience. Please ensure that it is
completed and also returned with your application:

ConceptShare is always expanding into new areas and technology partnerships at a rate that is
beyond our current capacity. Our current project is the management and reporting of our
innovation and business activity for a partnership with Corel Inc. Additionally we are involved
in Research and Development and recording of this activity for the next tax year is also part of
this project.

The salary for intern and employee benefits are eligible costs while there is an "Employer-
Employee Relationship." Such relationship exists when there is a verbal or written agreement in
which an employee agrees to work on a full-time basis for an employer for a specified period of
time, in return for salary or wages.

Any application not signed by proper authority of the company cannot be processed. The
undersigned certifies that all information provided to FedNor in support of this request for
funding is true and complete, and undertakes to provide any further information that may be
required for FedNor to render a decision, in a timely manner. The undersigned also herewith
provides consent for FedNor to make sufficient credit and other enquiries that may be necessary
in the evaluation of this request for funding.

2. STATEMENT OF WORK TO BE UNDERTAKEN (in point form)

The Statement of Work for the Youth Internship Program should clearly set out the following:

A. The objectives and tasks to be performed by the intern (the job description); and

B. What the specific task(s) will be, how the student will perform these tasks, the timing, and the
expected outcomes and measurements (the work plan)

The following is intended as suggestions for preparing these documents.

A. The Job Description:

1. Provide a statement of purpose or objectives for the proposed work term, detailing what the
interns role in the organization will be.

The role of Business Project Manager will involve the creation and management of reports and
documentation revolving around business activities in the areas of research and development
projects, and undertaken by ConceptShare. This role will also involve marketing and
distribution reporting to ensure proper adhesion to our guidelines.

B. The Work Plan: (in point form)

5. DESCRIPTION OF THE HIRING PROCESS AND PLANS FOR


SUPERVISING/MENTORING THE INTERN

a. Hiring process: Describe educational qualifications or areas of study, and details of other
specific areas of expertise or qualifications. Identify type of hiring ads, and sources to be used
(eg. Media or schools).
b. Plans for Supervision: Identify proposed mentors. Identify the form and frequency of
supervision by each mentor.

c. Anticipated impact and benefits for the Intern; particularly, long-term employment potential,
either with your organization or otherwise.

6. DESCRIPTION OF EXIT PLAN TO ASSIST THE INTERN IN SECURING


EMPLOYMENT AT THE END OF THE INTERNSHIP.

(ie: Intern will be hired by applicant or will be provided time to work on resumé, set up
interviews, etc.)

Please provide copies of your financial statements for the last fiscal year and the most recent
interim financial statement. Interim financial statements must be provided when the latest year-
end statements are six months or older.

8. PAYMENT OF FEDNOR CONTRIBUTIONS

FedNor will pay 50% of the approved costs of the project up to a maximum FedNor contribution
of $27,500 per intern.

Detailed instructions and claim forms will be provided to applicants after their projects are
approved by FedNor. However, applicants may find the following general information useful in
their financial planning.

In order to receive payment, applicants must file a claim. The claim must include a listing of all
the costs incurred for the project.

Applicants may claim 90% of the approved FedNor contribution during the implementation
phase of the project. The final 10% of the approved FedNor contribution will be released
following completion of the project, upon submission of the required final report. Applicants are
required to maintain proper books and records of the costs of the project, including payroll
records and cancelled cheques, and to provide FedNor auditors with access to these records when
requested.

Confidentiality: No commercially confidential information which you submit to us will be


disclosed unless otherwise specified by you; required to be released by law; or required by the
Minister of Industry to be released to an international or internal trade panel for the purposes of
the conduct of a dispute in which Canada is a party or a third party intervener. Information on the
