CODE OF CONDUCT
As a member of the Team Globalbike Charlotte, Globalbike Racing, Globalbike.org- (hereinafter GB), I promise to follow USA Cycling guidelines regarding rider conduct and rider sportsmanship. Furthermore, I will practice good sportsmanship and conduct myself in a professional manner at all cycling and non-cycling events where I am representing the Team. (Representing the Team is defined as any time you are wearing Team apparel, or participating in any race or training event where you are registered as a Team member, regardless of whether you are wearing Team Apparel, while participating at the event). Team members demonstrating a manifest unwillingness to meet the following Code of Conduct will be subject to suspension or dismissal from the Team. A. Unacceptable Behavior includes but is not limited to: o Verbal or physical abuse to any person at any time while representing the Team. o Use of foul language or physical abuse towards any person while representing the Team. o Making obscene gestures directed to any persons while participating in a racing or training event. o Malicious actions on or off the bicycle toward any persons, property, Team apparel and equipment while participating in a racing or training event. B. Team Apparel o As valuable advertising for the Teams sponsors, Team members are expected to wear the current, complete Team uniform whenever possible during training, and without exception while participating in all races. Team members are expected to maximize exposure for Team sponsors during racing and training events C. Safety o Team members must be conscious of their role as ambassadors to the non-cycling and recreational cycling community. Failure to wear a helmet and abide by traffic laws, if observed by the public, can be extremely detrimental to the Team and its sponsors, and may result in the revocation of all sponsorship benefits for the entire Team. o I agree to wear a certified helmet and obey all traffic regulations as enforced by all relevant local authorities while on individual, group or Team training rides, or as instructed during sanctioned events. D. Banned Substances o I will adhere to the World Anti-Doping Association and US Anti-Doping Association guidelines. o Team members known to take banned substances will be asked to offer a letter of resignation to the Team, effective immediately no exceptions. E. Organization Membership o I agree that in order to be a Team member, I must be a member in good standing with GB, must meet all requirements specified in this document, and must complete and sign a team application and all applicable waivers. o I understand that failure to obtain Team membership or meet requirements in this document will preclude me from racing as a Team member and from receiving Team uniforms or Team benefits. o I understand that failure to comply with the code of conduct or committing actions that bring discredit to the Team, its sponsors, or any of its associates, may result in dismissal without appeal. o I understand that annual renewal of membership is not guaranteed. o I understand that there is a $20 annual membership fee o I understand that I am required to purchase a minimum of one team jersey, bib-shorts, and socks for the season o I understand that I will be added to the Team Globalbike Charlotte Newsletter subscription list with the e-mail address supplied above. F. Racing License for Members who Race in addition to Ride Charity Events o I agree to obtain a valid USCF racing license from USA Cycling, Inc. (USAC), registered under the USAC Club name Globalbike Racing located in South Carolina IF I PLAN DO PARTICIPATE IN ANY RACES. You cannot be registered under any other team. o I understand that only one license is required for my primary discipline. I understand that one day licenses are acceptable for additional cycling disciplines and maybe be purchased on the day of the event. I understand that for USCF; I may ride under a 1 day license in Mens Category 5 or Womens Category 4. o I may ride under a 1 day license in Junior and Beginner classes for cycling events. o I understand that I am not eligible to participate in Championship events under a 1 day license. o USAT license requirements mirror the above that are applicable G. Racing Rules o I understand that I am responsible for reading, understanding, and complying with all rules and regulations for racing in events for which I have registered i.e. USCF rules for road events, I understand that failure to comply with said rules may result in my disqualification from the event, and possibly, dismissal from the Team. I. Race & Ride Commitment o As a Cat-4 or above race team member, we require a minimum 5 USCF or USAT sanctioned races, but ask that you target races specified by the category of your racing license. This is to maximize Globalbike in our market. o If I am categorized as a USCF Category 5 racer I agree to target as many races to get moved to Category 4 if more than 5 is required o If I am categorized as a USCF Category 3 or 4 racer I agree to target at least 5 races, including Crits, RRs, & Time Trials. o If I am a non-licensed rider, I agree to target at least two charity events, i.e.; 24 Hours of Booty. J. Team Meetings and Functions o Participation in Team meetings and functions such as ride events are encouraged. I agree to attend and participate in all Team meetings and functions unless otherwise excused by Team Director or other officers of the Team. o I understand that Team meetings and functions will be scheduled as far in advance as possible with at least one week prior notice by the Team Director or officers of the Team. o I understand that this organization must host at least one cycling event in accordance with USA Cycling Rules and that I am required to assist with at least some part of the promotion and/or execution of that event to include setup and tear-down of the venue.

WAIVER
IN CONSIDERATION of being permitted to participate in such activities as may now or hereafter be organized, operated, sponsored, and/or promoted by Team Globalbike Charlotte (hereinafter GB), the undersigned hereby represents and agrees as follows: 1. The forgoing release is intended to benefit and shall be deemed to extend to and cover, without limitation, the following entities or persons: Team Globalbike Charlotte or Team, its officers, directors, managers, employees, agents, contractors, and subcontractors, as well as any and all Team representatives, sponsors, municipalities, event directors and volunteers, their heirs, successors in interest, assigns and insurers (Releases); and, 2. I hereby acknowledge that the risk of injury from the activities involved in this program is significant, including the potential for property loss, serious bodily injury, permanent paralysis and/or death. Such risks include, but are not limited to, those caused by terrain, facilities, temperature, weather, equipment, vehicular traffic, conditions of athletes, lack of hydration, and actions of others, including, but not limited to, Releases, race participants, sponsors, volunteers, spectators, coaches, event officials and monitors, and producers or promoters of racing and training events. I also realize that various situations that may give rise to liability for serious personal injury and/or property damage may arise from the negligence or carelessness of the various persons and/or entities being released hereby, or from dangerous or defective equipment or property owned, maintained or controlled by them; and, 3. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS OF PARTICIPATING OR VOLUNTEERING IN SUCH ACTIVITIES, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, 4. I certify that I am physically fit, have sufficiently trained for participation in these activities and have not been advised otherwise by a qualified medical practitioner. I acknowledge that this Waiver and Release of Liability (hereinafter Waiver) may be used by event holders, sponsors and organizers, in which I may participate, and that it will govern my actions and responsibilities at said activities; and, 5. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the Company immediately; and, 6. I hereby consent to the provision of emergency medical treatment which may be deemed advisable in the event of injury, accident or illness which may occur while participating in any such activities. In the event of injury I further consent to the provision of such transportation arrangements as may be required in order to effect emergency medical treatment as well as the release of any and all information that may be required in order to provide reasonable and necessary medical treatment on my behalf; and, 7. I, for myself and on behalf of my heirs, next of kin, executors, personal representatives, successors, administrators, transferees, and assigns, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS the entities and/or individuals listed above (Releases) from any and all liabilities and/or claims, INCLUDING INJURY, DISABILITY, DEATH, or loss or damage to person or property which may result or derive in any way from my participation in the activities, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law; and, 8. I understand and expressly agree that the forgoing Waiver is intended and shall be construed to be as broad and inclusive as is permitted by the laws of the State of North Carolina, and that if any portion thereof is held invalid, it is agreed that the remainder of the Waiver shall, nevertheless, continue in full force and effect, without abatement of the remaining terms thereof. I further represent and warrant that I am over the age of 18 or have parental or guardian consent and that I have carefully read this Waiver and understand its terms. I understand that I am giving up important and substantial legal rights, including but not limited to the right to sue and/or recover legal damages from the individuals and/or entities released hereby. I further acknowledge and represent that I am signing this Waiver freely and voluntarily, with the intent to be bound by the terms of this Waiver to the fullest extent allowed by law; and, 9. I UNDERSTAND THAT THIS IS A LEGAL DOCUMENT AND THAT BY SIGNING IT ON PAGE 1 OF THE APPLICATION, I AM GIVING UP MY RIGHT TO SUE OR OTHERWISE MAKE A CLAIM against Releases and/or all other entities and persons described or otherwise listed herein; and, I HAVE READ THE RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ALL TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

WAIVER part 2
For good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I hereby irrevocably grant unto Team Globalbike Charlotte, Globalbike Racing LLC, or Globalbike.org are hereinafter referred to collectively GB, and the Teams assignees, licensees, sponsors, municipalities, and successors in interest, the right to use my photographic image and/or likeness, video and audio recordings, and name (hereafter Image) in all forms and for all purposes, including but not limited to Team advertising, trade, and/or commercial purposes. 1. I waive the right to inspect or approve versions of my image used for publication or the written copy that may be used in connection with the Image. 2. I hereby release Team and the Teams assignees, licensees, sponsors, and successors from any claims that may arise regarding the use of my Image including any claims of defamation, invasion of privacy, or infringement of moral rights, rights of publicity or copyright. Team is permitted, although not obligated, to include my name as a credit in connection with the Image. 3. Team is not obligated to utilize any of the rights granted in this Agreement. 4. I expressly agree that the forgoing Agreement is intended to be as broad and inclusive as is permitted by the laws of the state of North Carolina, South Carolina, and Florida - and that if any portion thereof is held invalid, it is agreed that the remainder of the Agreement shall, notwithstanding, continue in full legal force and effect. I am over the age of 18 or have parental or guardian consent. 5. I UNDERSTAND THAT THIS IS A LEGAL DOCUMENT AND THAT BY SIGNING IT ONE PAGE 1 OF THE MEMBERSHIP APPLICATION, I AM GIVING UP MY RIGHT TO SUE OR OTHERWISE MAKE A CLAIM against Team and/or all other entities and persons described or otherwise listed herein; and, I HAVE READ THIS UNLIMITED PERSONAL RELEASE AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

RECAP OF REQUIREMENTS TO BE CONSIDERED BEFORE SUBMITTING YOUR APPLICATION: 1. As a member of Globalbike Charlotte, you are required to; Pay your annual membership fee of $20 2. As a member of Globalbike Charlotte, you are required to ; Purchase of (at minimum) the following team kit items
a. Two S/S Jerseys b. Two Bib Shorts c. Two Pairs of GB Socks d. Other optional pieces may be available like vests, arm warmers, gloves, booties, tech-ts, and hats As a member of Globalbike Charlotte, you are required to; a. Purchase the official team helmet as determined by Athletix Elite Cycling Team -orb. Wear any brand of helmet white in color As a member of Globalbike Charlotte, you are required to race; a. As a Cat-4 or above race team member, we require a minimum 5 USCF or USAT sanctioned races. b. If you are categorized as a USCF Category 5 racer, you are required to target as many races to get upgraded to Category 4 (if more than 5 is required) c. As a member of Globalbike Charlotte, you are required to participate with the team in at least one local charity event As a member of Globalbike Charlotte, you are required to wear the current, complete team uniform at all times during training & racing - without exception. As a member of Globalbike Charlotte, you are required to; volunteer your time to any Globalbike specific events that we have to participate in to maintain our team status with USA Cycling As a member of Globalbike Charlotte, you might be asked on a rare occasion to volunteer in assisting with housing of our Globalbike Pro/Elite riders who might be racing in the local market. This allows the Elite team to keep expenses down so that more dollars can be used for Globalbike.org.
