Step Up to Social Work Application form guidance It is important that the guidance notes are followed when completing

the application form. These notes will help you complete the application form as effectively as possible and ensure all the relevant information is available to make a decision on whether you will be successful in the initial eligibility screen. General You will receive an email acknowledgement from CWDC once you have registered your email address on the first page of the application form. This enables you to use the link within the email to continue with your unfinished application if you do not complete it all in one go. Please note: There is a save button at the bottom of each page of the application form, you should only use this if you wish to return to your application at a later time. You will be given a unique reference number, please make a note of this as you will need to quote it in the event of any queries. You can only submit your application right at the very end of the application form on page 16, so you dont have to worry about submitting unfinished or unchecked forms. Page 1 (of 1) Pre-qualification questions (PQ1 PQ7) These questions relate to the entry criteria for the Step Up programme. You will not be able to submit your application if you cannot answer yes to all of these questions. You will need to provide documentary evidence to validate your statements if your application is successful in progressing through to the assessment centre stage. Page 1 (of 16) Contact Details (Q1 Q14) Please fill this page in fully. CWDC and/or regional partnerships will need to get in touch with you during the application process. Please let us know if any of your addresses will change during this time. Page 2 (of 16) Geographical Location (Q15) Please select your preferred choice of regional partnership where you would like to complete the programme. You have the option to also make a 2nd and 3rd choice, however please note, if successful at the eligibility screen your application will be sent to your preferred choice regional partnership.

Your 2nd and 3rd choices will only be acted on in the event there is a shortfall in candidate numbers in those areas. Each regional partnership is numbered the same as their local authorities that are listed in question 16, so you can easily pick which go together e.g. RP1 - West London Alliance has 8 local authorities prefixed with RP1. (Q16) Please also state a preference for a specific local authority within each of your Regional Partnership options. Please note that if you are successful, it cannot be guaranteed that you will complete Step Up in that local authority. Page 2 (of 16) Personal Information (Q17 Q20) Please answer all of these questions as we need to know if you are eligible to study/work in the UK for the duration of the programme. If you are successful, you will be registered with a University and as part of this process, the University will need to know whether you would be classed as a Home / European Union / International student. You are likely to be assessed as a Home /EU student if: You have settled status in the UK or EU and you have lived in the UK or EU for the three years immediately before the start of this course and your main purpose for living in the UK or EU was not for full-time education in any of these three years.

If you do not meet all of these criteria you may be assessed as an international fee-paying student and therefore responsible for paying your own University fees. Page 3 (of 16) Personal Information (Q21) Please indicate if you hold a full UK driving license. Please see the statement on the application form for more guidance. (Q22) You must tick one of the boxes in question 22 (a & b); All candidates will undertake an enhanced CRB check prior to any formal offer of a place on Step Up being made. Page 3 (of 16) Education secondary (Q23) Please fill this section in fully. The GCSE grade C or above, in Maths and English or equivalent is part of the eligibility criteria. If you have not yet achieved them, please put the date you will achieve your results in the 'Dates To' box and enter 'TBC' (to be confirmed) in the 'Grade' box. You will need to have achieved both by the time the course commences in February

2012 (certificates will need to be produced as evidence of achievement) . To note: If this section of the application form is left blank your application will not be progressed further. Qual. Type means GCSE etc. Page 4 (of 16) Education Secondary continued (Q24) Please fill in details of any vocational qualifications you hold. Awarding body means the organisation that awarded your certificate e.g. City and Guilds, AQA. Assessment Centre is the place where you received your training e.g. your College or NVQ centre. (Q25) Please fill in details of any A/A-S level or equivalent qualifications. Page 5 (of 16) Education (Undergraduate Degree Course) (Q26 32) This section must contain full details of your degree, where studied, dates and class of degree achieved. To note: If this section of the application form is left blank your application will not be progressed further. Page 5 (of 16) Education (Postgraduate Degree Course if applicable / Professional Qualifications / Members) (Q33 39) This section only applies if you have a postgraduate degree (Q40) Please fill in any details of membership of any professional associations or, any additional professional qualifications that you hold. Page 6 (of 16) English Language (Q 41) This refers to any examination or qualification you have undertaken in spoken English. (Q 42) Most qualifications will have been recorded in previous sections of the form. This question refers only to those qualifications that do not fit easily into the other categories. . Page 7-10 (of 16) Employment History (since leaving full time education)

(Q43 Q49) Please ensure this section is completed in full as it is a prerequisite for any application for Social Work to have your full employment history accounted for. This should also include any gaps in employment e.g. travelling, not in paid work. Page 11 (of 16) Additional Information (Q51) Please use this space to tell us about any additional experience, training or further qualifications you have not had the opportunity to demonstrate so far on the application form, or any other information that you feel may help your application. Page 11 (of 16) References (Q52) This section must include two contacts you would like us to use as referees (this must include your current or last employer and, an academic referee from the last course you attended). Friends or relatives are not acceptable contacts for references. To note: your application will not be progressed if this section is not completed. Page 12 (of 16) - Competency Based Questions (Q54 Q56) These questions are mandatory and the information contained will be used by regional partnerships to make decisions about which candidates will be invited to the next stage of the selection process. Please think about your answers carefully and ensure each question is answered clearly and in full. Page 13 (of 16) Personal statements in support of your application (Q57) This section is optional and you can use this space to add any additional information in support of your application. (Q58) Please tick the check box to confirm the information you have supplied as being true to the best of your knowledge and belief. To note: your application can not be submitted if this box is left unchecked. (Q59) Please advise if you have a disability that would require you to have some assistance at interview. Page 16 (of 16) Submit page This is the final submission page of the application form Please make sure you have: Printed or saved a PDF copy on your computer so you can refer to it if you are invited to an assessment centre. Filled in all the fields that require it. Your form may not get through the first stage process if you dont.

The next stage is that CWDC will check your form to see if you are eligible for the Step Up programme. Please note: We are anticipating a high volume of applications and would therefore request that after you have submitted your application you only contact either CWDC or the regional partnerships if you havent heard anything by the dates detailed below: If you are UNSUCCESSFUL in the eligibility sift: You will receive an email from CWDC by the end of September 2011 informing you of such. If you are SUCCESSFUL in the eligibility sift: you wont hear from us, so please presume your application has been passed to your preferred regional partnership. Your preferred regional partnership will contact you by the 21st October 2011 to let you know if you have been either successful or unsuccessful in the second stage of the application process. If you are successful, they will either invite you to an assessment centre to be held during November 2011 or, with your permission, place your name on a reserve list in the event that additional places become available.
