CTOS ID (CID) REGISTRATION Purpose of CID registration: a.

) To notify you of new information and updates to your CTOS Report b.) To enable you gain effective control as to who may receive your CTOS report c.) To enable you get your CTOS Self Check Report from home/office via the Internet at any time d.) To faciltate your easy and direct communication with CTOS regarding your CID and/or CTOS Report e.) To get your applications and transactions with third parties processed quickly and expeditiously TERMS & CONDITIONS OF CID REGISTRATION By this registration, I am/We are fully aware and agreeable to the following: a.) CTOS can retain, process, use my/our information that I/we have provided to CTOS in my/our CID profile now or at any time in the future. b.) CTOS may provide information in my/our CID profile to party/ies which I/we have consented to receive my/our CTOS Report. c.) CTOS may communicate with me/us via any form of electronic messaging ( including email, sms, etc ) regarding my/our CID and /or CTOS report and such shall be deemed good and valid means of communication between CTOS and me/us.

I/We further warrant the contact particulars given to CTOS are valid until I/we change them by advising CTOS in writing or as updated by me/us in my/our CID profile via the CTOS-CID web site (http://ctosid.ctos.com.my or the applicable web site, from time to time.) Important Note: In the event of ambiguities, conflicts or possible variations in interpretation between the English version and other languages of this document, agreement, website or brochure, etc the English version shall always prevail. PENDAFTARAN CTOS ID (CID) Tujuan pendaftaran CID: a.) Anda akan dimaklumkan apabila CTOS memproses maklumat baru mengenai anda. b.) Anda berhak menentukan sesiapa yang boleh menerima laporan CTOS anda. c.) Anda boleh mendapatkan laporan semakdiri CTOS anda dengan mudahnya melalui internet di rumah/pejabat pada bila-bila masa. d.) Anda boleh menghubungi CTOS berkaitan laporan CTOS atau CID anda dengan mudahnya. e.) Proses urusniaga anda dengan parti yang berkaitan boleh dicepatkan. TERMA & SYARAT UNTUK PENDAFTARAN CID Dengan permohonan CTOS Identiti (CID) ini, saya/kami sedar dan bersetuju seperti berikut: a.) CTOS boleh menyimpan, memproses, mengguna maklumat saya/kami yang telah saya /kami berikan kepada CTOS di dalam profil CID saya/ kami pada masa sekarang atau pada bila bila masa di masa depan b.) CTOS boleh memaparkan maklumat di dalam profil CID saya/ kami kepada pihak /pihak-pihak yang saya/kami bersetujui untuk menerima laporan CTOS saya/kami. c.) CTOS boleh berkomunikasi dengan saya/kami melalui pelbagai jenis mesej elektronik (termasuk emel, sms, dll) mengenai CID saya/kami dan / atau laporan CTOS dan itu akan dianggap sebagai cara komunikasi yang sah antara CTOS dan saya /kami. Saya/Kami mengakui bahawa butir-butir hubungi saya/kami yang diberikan adalah butir-butir hubungi saya/kami yang sah sehingga saya/kami menukarkannya dengan memberitahu CTOS secara bertulis atau yang dikemaskini oleh saya /kami melalui laman web CTOS-CID (http://ctosid.ctos.com.my atau laman web yang berkenaan , dari masa ke semasa) Notis Penting: Versi Bahasa Inggeris akan digunakan sekiranya menghadapi sebarang kekeliruan, konflik atau kemungkinan variasi antara Bahasa Inggeris dengan bahasa-bahasa lain pada masa menafsirkan dokumen ini atau kontrak, laman web, risalah dan sebagainya.
