Quality of Care of Reproductive Health in China Today

Dr. Zhao Baige National Population and Family Planning Commission of China(NPFPC) Background Basic data of population in China in 2000: Population is 1.266 billion; Growth rate is 8.77; Total fertility rate is below than 2; Average life expectancy is 69.36 years for male and 73.11 years for female. The family planning network has covered 95% of China's population in China. There is a MCH system undertaken by the Ministry of Health in China, but majority of its exists only in the cities' areas. There are 4,000,000 family planning cadres and full-time workers in the nation, not including part-time workers in villages. Table1 Family planning network in China in 1999

In China, the main contraceptive methods are IUD and female sterilization, while condom, pills, implant and spermicides are seldom adopted, which means that women of childbearing age play a key role in practicing family planning in the country. History of Quality of Care for FP/RH in China. In 1995, the SFPC decided to carry out a pilot test for the quality of care in China. In 1996, the pilot test for the quality of care began to be initiated in the country. In 1997, with the continuation of the pilot test, international organization participated in QoC of China, for example, at the workshop on the quality of care in Yanchen, Jiangsu province. In 1998, the SFPC started international cooperation with the Population Council, Ford Foundation, and University of Michigan. In 2001, the Quality of Care program has served 827 counties in China. Reducing the risk factors associated with major causes of disease and the key threats to human health arising from environmental, economic, social and behavioral causes.

Developing a health system that is managed to ensure equitable health outcomes and cost-effectiveness, as well as a response to people's legitimate needs. It is financially and procedurally fair and can encourage public involvement. The State has promoted an effective health program and formulated relevant policies for socioeconomic development. Political environment A decision has been made by the State Council to improve China's population program to ensure a stable and low fertility level. A White Paper: China's Population and Development in the 21st Century. hina will work out the Law on Population and Family Planning. The State Family Planning Commission will formulate the Regulations on Family Planning Technical Services. Changing strategy for population and family planning program China is bringing about or has brought about some changes in the strategy for its population and family planning programs with its focus switched: from family planning program to reproductive health; from population control to client-centered services; from mere contraceptive methods to informed choice; from administrative order to providing quality of care services. The Significance of QoC for FP/RH It is an important practice to China's family planning program and a follow-up to ICPD. It poses a major innovation for family planning and reproductive health programs. It proves to be a concert action in achieving the goal to serve the people. The population is at the top of the triangle, that means family planning quality of care is client-centered. Population is at a more important place. Technical services and the service system are at the lower places as two supporting points, which means technical service and the service system are the bases for providing family planning QoC. Delivery of Quality of Care The SFPC pays great attention to improve the legislative system and to guarantee the rights of clients. Provide sufficient and available information to clients. The standardized operational process and the service system to meet the needs of the population of childbearing age were established. The technical competence of service providers was improved. A comprehensive system of evaluation and feedback was developed. Achievements of QoC in China The QoC program has helped population and family planning workers to have a clear understanding of the direction and approaches for family planning implementation. 827 counties/districts have been set up the QoC program in China. A large group of program managers and technical providers have been trained. The concept, ideals and approaches of QoC have begun to be accepted. A network for international cooperation has been set up. It is an important component of the follow-up of ICPD. International Organizations' recognition of QoC in China "One of the most exciting developments, post-ICPD, has been the new service-oriented approach to family planning being promoted by the SFPC". "The pilot represents a large conceptual change for China". --- Ford Foundation "The SFPC quality of care project is becoming a widely recognized example of change and experimentation in the world's largest family planning program". "The Population Council looks forward to continuing its collaboration with the SFPC". --- Population Council "China tries easing once-brutal approach to family planning".
