Rebate offer is good only on qualifying products. Offer valid on purchases made between April 1 - May 31, 2012. Upon fulfilling the requirements below, Tenneco will send you a rebate check. 2) You must submit the following items to receive your Monroe Brakes Brake Bonus rebate check: a) Authentic dated sales receipt (copy or original) with business identification verifying the purchase of the qualifying products. The purchased product part numbers MUST be identified on the sales receipt. b) This form, filled out completely and legibly. Visibly altered documentation will not be honored. 3) Bonus! In addition to your Brake Bonus rebate check, you will receive a Monroe Shocks and Struts $50 BACK certificate, good for your next purchase of qualifying Monroe ride control products. Certificate dollars will be a mail-in reimbursement. 4) Send this completed form and required proof-of-purchase (dated sales receipt) to the address indicated below.
