APPLICATION GUIDELINES ON REGISTRATION AS SAFETY AND HEALTH OFFICER TRAINING CENTRE

1.0

INTRODUCTION

Regulation 6(1), of the Occupational Safety and Health (Safety and Health Officer) Regulations, 1997 states that a person who wish to register himself as a safety and health officer at least should have qualifications as follows :i. Diploma in occupational safety and health or equivalent from any professional body or institution approved by the minister on the recommendation of Director-General; successfully completed training in occupational safety and health course and passed any examination for the course or equivalent, that is approved by the Minister on the recommendation of Director-General and had a minimum of three years experience in the field of occupational safety and health; Have at least ten years working experience in the field of occupational safety and health; or Have other qualifications or have received training from time to time set by the Minister according to subsection 29(4), of the Occupational Safety and Health Act 1994.

Therefore, every applicant who wishes to conduct the course should register with the department before commencing the course. 2.0 PURPOSE

Provide guidance to the applicant, on the procedure how to obtain registration with the department as the safety and health officer training centre. 3.0 REGISTRATION APPLICATION Each application for registration should be sent to :Director General, Department of Occupational Safety and Health, Level 2,3 & 4, Block D3, Complex D, Federal Government Administrative Centre, 62530 Putrajaya. DEFINITION Safety and health training centre Company or organization conducting a safety and health officer course accredited by the Department. In-house instructor / trainer A person employed full time permanently to teach in the safety and health officer course at the prescribed training centre. Invited instructor / trainer A person employed part time to teach in the safety and health officer course at the prescribed training centre

5.0

APPLICATION DOCUMENTS An application shall be enclosed with the following particulars : i. ii. iii. iv. v. vi. vii. viii. ix. x. xi. xii. xiii. xiv. Application letter Duly filled training centre registration form( form I ) Company background, organization chart and safety and health policy List and particulars of course instructor / training (form II) including C.V Copies of certificates with respect to topic that is going to be taught Copy of ROC certificate Course module including time allotted for each topic Complete course notes Lesson plan Presentation slides Participants evaluation system and participants feedback form Control procedures of the issuance of certificates and participants record keeping Copies of relevant agreement letter Supporting documents

APPLICATION LETTER Application letter must be enclosed with full details on the course and the purpose of the course conducted

5.2

TRAINING CENTRE REGISTRATION FORM Applicant is required to fill in particulars in the training centre registration form (form I) and submit together with the application.

5.3

COMPANY BACKGROUND AND ORGANISATION CHART Application shall be enclosed with background and company profile stating the total number of staff, companys main task, organization chart and the management system in general.

5.4

COURSE INSTRUCTOR/TRAINER a. b. All Instructors/trainers either in-house or by invitation, shall have written approval from the Department; For instructors/trainers employed by invitation, an agreement letter made between the training centre and the instructor/trainer shall be for a minimum period of not less than two (2) years. The following information shall be enclosed in the application : i. ii. iii. List and instructors/ trainers particulars (form II) Copies of personal biodata (curriculum - vitae) Copies of the following certificates :a. Copy of the train the trainer course certificate b. Copy of the certificate of related topic that will be taught. Instructor/trainer appointment letter Acceptance letter from instructor/trainer Copy of agreement letter between the training centre and instructor/trainer (if the instructor/trainer employed by invitation) Copy of EPFs payment slip (for in-house instructor/trainer)

Note : Copies of related certificates shall be certified or hand in original certificate to the Department for verification

COURSE MODULE Applicant shall prepare course module stating main topics for each module and also sub-topics which are given specialisation and also total hours allocated. The syllabus should be at least to cover all the topics as listed in attachment A1.

COURSE NOTES

Applicant shall submit a complete set of course notes as what is going to be given to the course participants. The notes should cover all the topics as specified in the course module. 5.7 LESSON PLAN Lesson plan shall be prepared based on prescribed course module. It is going to be used by the instructor/trainer as a guide during conducting the course. Please use the format as in attachment A2. 5.8 PRESENTATION SLIDES Presentation slides shall be prepared based on the prescribed module and on the prepared course notes. It is going to be used by the instructor/trainer during the course. Presentation slides must be prepared either in Microsoft power point or transparency. 5.9 PLACE/ PREMISE FOR THE COURSE TO BE CONDUCTED i. Applicant is required to state the address of place/premise where the course is going to be conducted together with the list of teaching facilities and equipment. The approval will be based on the reports of premise inspection which will be conducted at the place/premise. Place/premise applied for the training centre should comply following conditions : a. the

The place/premise must be owned or rented full time and its management and administration shall be under the full control of applicant completely for the period of approval; Closed lecture room, should have adequate lighting and ventilation as well as enough space area and comfortable for capacity of 15 participants ; Suitable tables and chairs should be used for the classroom; Application of teaching equipment equipment, blackboard, etc.; such as audio-visual

ii. If the place/premise for conducting the course is not belong to the applicant or is only rented, copies of the following documents shall be attached together with the application :a. A copy of duty-stamped agreement letter between applicant and the owner of the place/premise for the usage of the place by stating the agreement period and other conditions agreed upon; and Copy of ownership document containing name of the owner of the place/premise, also need to be enclosed.

5.10 PARTICIPANTS COURSE EVALUATION, PRACTICAL TRAINING AND FEEDBACK Appropriate measures should be taken to prepare participant for the examination at NIOSH. Further to this, the applicant should prepare and submit the following particulars to the Department : i. evaluation system and method their level of competency for participants aiming to measure

ii. Training provided to the course participants for the purposes of exposure to the actual working environment ; and iii. Course feedback by the participants about the course attended and instructors/trainers performance. 5.11 RECORD KEEPING OF COURSE PARTICIPANTS i. All the records of participants consisting of the following information shall be kept safely and should be submitted to DOSH if requested:a. b. c. d. e. f. h. ii. Participant name; Identity card number; Date of the course held; Results of the examination taken at NIOSH; Serial number of attendance certificates; Evaluation record of each participant including the assignment report or written test; Course feedback form by each participant.

All documents related to participants and training centre shall be kept in the headquarters.

5.12

SUPPORTING INFORMATION Applicant is required to submit supporting documents explaining how course handling and management are conducted including document for participant and instructor/trainer evaluation system, pamphlets about the courses, including examination system and entrance qualification conditions. Applicant may submit any accreditation or testimonial received from any organisation or professional bodies (if any).

6.0

REGISTRATION PERIOD For applicant who has fulfilled the prescribed conditions, the registration will be granted for a maximum period of two (2) years from the date of the approval letter is issued.

Note :

Applicant is responsible to ensure that any agreement or contract has been made does not expire during the given approval period.

7.0

EXTENSION OF REGISTRATION Application for extension of registration shall be made three (3) months before expiring date. The application should be enclosed with the following particulars:i. ii. iii. iv. All the relevant agreement letters or contracts

Reminder

Training centre which has an expired registration is not allowed to run the course.

8.0

SUSPENSION AND CANCELLATION OF REGISTRATION The Department may suspend or revoke the registration as training centre if any prescribed conditions stated in the approval letter or other conditions issued by the Department from time to time has been violated. The registration also will be null and void if the training centre fails to submit an extension application for registration within the specified period.

9.0

BRANCH OF TRAINING CENTRE The training centre is not allowed to conduct courses at place other than the place as stated in the approval letter. However, the training centre may submit an application to DOSH requesting approval for opening a new branch.

9.1

APPROVAL OF BRANCH Applicant who is already registered for safety and health officer training centre and wish to open a branch shall submit written application to the Department of Occupational Safety and health (DOSH).

9.2

ADDITIONAL INFORMATION AND DOCUMENTS The following particulars shall be submitted for branch application: i. ii. Application letter; Particulars of training centre and instructors/trainers(please fill in form I and form II) iii. Branch organisation chart iv. Copies of all agreement letters with duty-stamped between training centre and parties concerned.

9.3

ADMINISTRATION AND OPERATION MANAGEMENT For the opening of branch training centre, the following matters shall be under the responsibility and full control of head office management :i. program arrangement during the course.
