GUIDELINES IN PREPARING BPS FORM 18-A (PRESCHOOL/ELEMENTARY/SECONDARY PUPILS/STUDENTS PROMOTION FOPR S/Y 2012-2013) 1. Curriculum spe3llout not acronym !st & 2nd Year Secondary Education Curriculum 3rd & 4th Year Basic Education Curriculum 2. Curriculum Year Write in full words: First Year, Second Year, as the case may be. 3. School spell out not acronym 4. Municipality spell out, do not abbreviate 5. Date inside 18-A should be March 16, 2012 for Fourth Year/ Grade 6 and March 30, 2012 for the undergraduates. 6. Surnames first, listed alphabetically, males separate from females. Use another form of 18-A for females, if all names cannot be accommodated in one form. 7. Write Male inside the box where you find the word NAMES 8. Immediately on the first line under names write no. 1 as your first entry of male 9. If you have few enrollees, leave five (5) lines. On the 6 th line the word Female. Immediately on the next line, write your first entry of female. 10. If you plenty of enrollees, disregard the leaving of five (5) lines. You can away with writing of Female. Immediately after your last entry of male, write no.1 for your first entry of female. 11. If you have more than fifty (50) students in class, use separate 18-a for male and female. 12. Please indicate asterisk (*) before the number of names of students with discrepancies of the total number of units. On the bottom of the column for names, indicate the legend explain why there is a discrepancy on the total number of units as guide for checkers. 13. Under Home Address write the full name of barangay, town, you can just abbreviate Camarines Sur as Cam. Sur. Please break the barangay, town and province with comma. 14. Under the Years in School write the no. of years the student has attended from grade one up to the present, including repetitions, if any. 15. Under Age write in decimal numbers 16. Total Number of Days of Attendance in Curriculum Year refers to the actual attendance of every student but not exceed 202 days except for repeaters of Fourth Year. 17. Final Rating should be stated in multiples of one(1) or whole numbers. 18. Write the curriculum year in every subject area, e.g. English I, Filipino II etc. 19. Action Taken Write P for Pumasa(in black ink) and HP for Hindi Pumasa(in red ink) 20. Credits earned in Previous years Write the total credits earned. If one is in the First Year , write zero(0).

21. Add credits/units earned to the current year to get the total . Below are the corresponding unit credits per subject area and the total units earned for the current year. English 1.5 units Science 1.8 units Total units earned for the current Year: Mathematics 1.5 units First Year 10. 2 units Filipino 1.2 units Second Year 10.2 units Makabayan Third Year 10.5 units Araling Panlipunan 1.2 units Fourth Year 10.8 units Edukasyon sa Pagpapahalaga First Year .6 unit Second Year .6 unit Third Year .9 Fourth Year .9 TLE 1.2 units MAPEH 1.2 units CAT(Fourth Year) .3unit 22. Under Promoted or Retained write in full word Pumasa for thosew who are being promoted and Ipinapanatili ( in black ink) for those being retained( in red ink) 23. Students who lack three (3) units or more should be retained 24. Students in Fourth Year who lack even one (1) or less unit should be retained. This should deprive the students from joining the graduation rites. If unit is acquired after Summer Class, student is considered a graduate during summer not within previous school year. 25. Students to be considered passing should have a rating of 75 and above. 26. Total age of students, both male and female should have totals. Those who dropped should have age deducted showing the computation and the difference should be stated. This will facilitate the checking. 27. Grand total of age of students should be indicated on the space provided under the column for AGE 28. Every subject teacher should initial at the bottom of the subject taught. 29. The teacher preparing the report on promotion should sign over five (5) spaces from the last line at the bottom on the right corner of the Form 18-A in all sheets. 30. At the back of the Form 18-A in line with the curriculum, spell out !st & 2nd Year Secondary Education Curriculum 3rd & 4th Year Basic Education Curriculum 31. Date of close of school year will depend on the school calendar of every school but in general, it should be March 30,2012. 32. Date diploma issued to the graduates for fourth year is the date of graduation. For the undergraduates, spell out Not Applicable. 33. Date of Submission of Form 18-A should be March 23, 2012 for graduating students and April 9, 2012 for the undergraduates. 34. For respectfully submitted signature of School Head over typewritten name. 35. Name under Approved: By Authority of the Schools Division Superintendent:
