Want to learn how to prepare for college? Interested in meeting students from other San Antonio high schools?

Excited to be an advocate for college preparation for your peers at your school? Then Camp College is for you! Camp College is a free, two-day, overnight camp at Trinity University June 28-29, 2012 put on by GenTX San Antonio and Project WORTH. Campers will learn about potential careers, how majors can fit into careers and about the different types of schools to enroll in. Interactive workshops will help hone campers writing skills and will prep students for admissions interviews. Since students will stay on campus during the two-day event, they will get a true taste of the college lifestyle! Campers will eat meals in the on-campus dining facility and will spend the night in one of the residence halls located on the Trinity University campus. Parents will also be invited to share a meal with their students and get college prep information geared towards them during our parent college session. Students will leave camp armed with the knowledge needed to make decisions about their future beyond high school and will be able to encourage their peers to pursue college opportunities as well. Camp College is a two-day, overnight camp. There is NO TUITION for Camp College. Camp College includes: Breakfast, lunch and dinner on Thursday, June 28 th Breakfast & lunch on Friday, June 29th Overnight accommodations (including linens) Training to become a GenTXpert college advocate Workshop sessions about a variety of college preparation topics Teambuilding activities with students from area high schools

All interested students must complete the Camp College Application and Accident Waiver and Release of Liability forms below. Applications are due Friday, June 8, 2012 by 11:59pm, CDT. Applications should be sent via email to aetemadi@gentx.org, via fax to 210-254-9406 or mailed to GenTX at 150 W. Sunset, San Antonio, TX 78209. Students will be notified of their acceptance into Camp College by June, 15, 2012 and participating families will receive a detailed information packet. Camp College applicants must meet the following criteria: High school junior or a high school senior in the 2012-2013 school year Enrolled in a high school in the San Antonio area Desire to learn steps to prepare for college Desire to meet students from around San Antonio Willing to serve as an college advocate for their peers throughout the 2012-2013 school year

Name of the Activity or Event: Camp College Date of Activity or Event: June 28-29, 2012 I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING AND/OR VOLUNTEERING IN THIS ACTIVITY OR EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity or event in which I may participate, and that it will govern my actions and responsibilities at said activity or event. I recognize and am fully aware that facilities or events may involve physical risks of injury to my child. I am fully aware of, understand and voluntarily accept those risks, including the risk of automobile or vehicle accidents, and agree that neither Generation TX San Antonio (GenTX SA) nor any of its employees, officers or agents will be liable for any personal injuries (including, but not limited to, bodily or mental injury or death), property loss, economic loss or any other damage resulting from any acts or omissions, including the active or passive negligence of GenTXSA, its employees, officers or agents, including driver negligence. In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: (A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: GenTX SA and/or their directors, officers, employees, volunteers, representatives, and agents, the activity or event holders, activity or event sponsors, activity or event volunteers; (B) I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity or event, whether caused by the negligence of release or otherwise. I acknowledge that GenTX SA and their directors, officers, volunteers, representatives, and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific event or activity on behalf of GenTX SA. I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity or event. PARENT / GUARDIAN WAIVER FOR MINORS (Under 18 years old) The undersigned parent and natural guardian does hereby represent that he/she is, in fact, acting in such capacity, has consented to his/her child or wards participation in the activity or event, and has agreed individually and on behalf of the child or ward, to the terms of the accident waiver and release of liability set forth above.
