2012-2013 Agreement Form Please read through this form with your child and return with child and guardian signatures by Friday, August 31. I. Student Handbook Agreement: I have read and understand all of the information in this handbook. I will do my best to follow the rules and guidelines stated. II. Parent/Guardian Handbook Agreement: I have read this handbook and have noted the required rehearsal and performance dates for my child. III. Student Transportation Agreement: This is to certify that I am willing to abide by the rules and regulations of the Eanes ISD and Hill Country Middle School; that I will accept all directions and suggestions given by any of the sponsors while on the trip; that I will abide by any rules which the sponsors find necessary to make during the trip; and that I will accept responsibility for my actions during the trip. I promise that I will conduct myself in a manner that will show pride ingroup, the Eanes ISD, and myself.

IV: Parent Transportation Agreement I certify that I am the parent or legal guardian of the student named above. I approve of, and agree to the conditions, rules, and regulations outlined above. I hereby authorize the sponsors to engage the services of a physician and/ or hospital for emergency treatment of my child while on the trip if the need should arise. I agree to bear the cost of such treatment rendered. I will not hold the teachers, sponsors, Hill Country Middle School, or Eanes ISD responsible in case of illness or accident. Any medical information needed about my child has been described on the reverse of this form. V: Permission for Emergency Treatment This form enables your child to receive treatment if taken to the hospital by school personnel or a sponsor. I, the undersigned, do hereby authorize officials and sponsors of Hill Country Middle School to contact directly the person named on this document, and do authorize the named physicians to render such treatment as may be deemed necessary in an emergency, for the health of said child. In the event physicians, other persons named on this form, or parents cannot be contacted, the school officials are hereby authorized to take whatever action is deemed necessary in their judgment, for the health of aforesaid child. I will not hold the school financially responsible for emergency care and/ or transportation of said child.
