MISSION STATEMENT
As a ministry of New Testament Baptist Church, New Testament Baptist Christian School is dedicated to assisting Christian parents in fulfilling their God-given responsibility of training their children by providing a godly atmosphere that equips students to grow both academically and spiritually in order to serve God effectively in their home, church, and community.
New Testament Baptist Christian School is a Christ-centered faith-based ministry. As such, it meets its financial responsibilities through the financial faithfulness of its students families, financial contributions from concerned individuals and prayer partners, and the New Testament Baptist Church. New Testament Baptist Christian School receives NO subsidies or grants from other sources nor do we seek them. Therefore, the Schools ministries are dependent upon the timely fulfillment of each familys monthly financial commitment. We believe that the tuition and fees are an investment in your childs education and spiritual formation. The Board of Directors (the Pastor, New Testament Baptist Christian School Principal, and the New Testament Baptist Churchs Trustees) accepts responsibility for establishing policy concerning the amount of tuition, the manner of payment, and in general, the development of policy in this area of concern. Furthermore, it is the responsibility of the Board to insure that adequate financial resources are available for the school and that enrollment is as available and affordable as possible to all families. It is vital that all NTBCS families understand their financial commitments to the school and faithfully pay all tuition and other fees on time.

APPLICATION INSTRUCTION
The enrollment process helps NTBCS ensure we are partnering with families desiring to assist us in fulfilling our philosophy and purpose as stated above, and also enables us to assess your childs aptitude to succeed academically. In order to successfully complete the enrollment process for your child, the following items are required. All required forms are en-closed. This entire process must be completed in order for your child to be considered for enrollment. Your prompt response to these items will help us process your application in a more efficient manner. Step #1: Completed Application Returned to NTBCS with $75.00 Application Fee per Student. (Check list) Parent Information Application Student Application Enrollment Contract Authorization to Release Transcripts (Excluding Pre-K3 and Pre-K4) This form should be presented to your home school if your child is transferring from another school. Your childs school records, to include 2 years of reports cards and 2 years of Standardized Test Score Results, are required to complete the application. Copy of Certified Official Birth Certificate (Please present a copy, as originals will not be returned.) Copy of Up-to-Date Immunization Record or Parental Refuse to Vaccinate Form Any other Health, Medical or Psychological Reports Student Medical Release/Emergency Contacts Form (1 Per Student Applying) Student Pick Up Authorization Form.

Step #2: Admission Screening/Testing The applicant will participate in a screening and/or take an appropriate grade level test, depending on grade entering. The testing will be scheduled after all required paperwork has been submitted. Step #3: Parent/Guardian School Interview Once your childs Application is completed (completion of Step #1 and Step #2), you will then be scheduled for an interview with an NTBCS Official. New Testament requires that the parent(s)/guardian(s) of the applicant and the applicant meet with the Administration. This meeting will be scheduled after all required paperwork and admissions screenings and testing is completed.

The school operates solely on student tuition, fees, and fundraisers. It is critical that all charges be paid completely and promptly. On the basis of this enrollment contract, the school incurs financial obligations for faculty, staff, facilities, and supplies that span the entire year. We expect, in good faith, that your child(ren) will attend NTBCS for the entire school year. Please read over this entire contract, check the appropriate boxes, initial items indicated, and sign at the bottom of the page. 1. Tuition payment plan: Check your tuition payment plan choice. Payment Options: All families shall be expected to make tuition payments according to one of the following payment plans. Each familys preferred manner of payment must be submitted each year at the time of student registration. Options for payment shall include: Payment-In-Full (PIF): Under this plan the entire amount of tuition is paid on or before July 1st, or at the time of registration if a later enrollee. This payment is made directly to the school office. Deferred Monthly Payments: Under this plan, the entire amount of tuition is paid monthly over a twelve (12) month period (June May), or a ten (10) month period (July April). All charges incurred on this account must be paid. NTBCS must be notified immediately of any circumstances that will affect the financial status of the family from fulfilling the agreement. Each payment must be paid on or before the 10th of the month. For later enrollees, payments can be prorated over the remaining months in the payment year, with all payments being completed no later than May.

Without such information, the following policy will apply when tuition payments are received late. A. Full Payment. When full payment has not been made by July 1st, the account will be assessed a $100.00 late fee. In addition, all tuition payments shall then be made through the deferred monthly payment plan. B. Deferred Monthly Payment Plans. Accounts which continue to be past due by the 20th of the month will be charged a $25.00 late fee and receive a call from the School Office. Parents who find themselves in financial difficulty are urged to contact the School Office as soon as possible. Accounts which remain past due by the end of that month will receive a second late fee assessment of $25.00. If payment is not received by the 15th of the following month, a withdrawal conference could be scheduled with the School Board. C. Delinquent Accounts. School families who have missed two monthly payments and have not made suitable arrangements within fourteen (14) calendar days of the second missed payment will be informed that their child(ren) will not be re-admitted to school nor will they be allowed to register for the next year. D. Delinquent Tuition from Previous Year(s). All previously unpaid tuition and fees must be paid by August 1st if a student is to be readmitted on the first day of class for the new year. Payments are to be made directly to the school. If payment is not possible, suitable arrangements must be made with the School Board. E. Penalties for Persistent Delinquent Accounts. New Testament Baptist Christian School reserves the right to impose appropriate penalties in those situations where delinquent tuition problems persist. These may include, but are not limited to, denial of re-enrollment, suspension of students, not allowing students to sit for exams, restricting student participation in extra-curricular activities, withholding of yearbooks, and withholding of transcripts and records as prescribed by law. F. No post-dated checks will be accepted. G. Returned Checks Checks returned to the school for insufficient funds, closed account, etc. will result in a $30.00 NSF fee. The family will be contacted by the School Office upon notification of the returned check. Returned checks MUST be replaced with cash within a five day period of the return notice. If two (2) checks are returned to the school for any reason during the school year, all future payments MUST be made by cashiers check, money order, or cash. Note: If the due date falls on a weekend or holiday, the payment will be considered timely if received on the next business day. Students will not be allowed to continue school if payment arrangements are not made with the school. 3 NSF payments will require future payments to be made in the form of a cashiers check. Tuition and fees are nonrefundable, except as outlined in this contract.

2. Internet Agreement: Part of the program of studies at NTBCS involves the use of computer technology including Internet use. I understand that my child will use the internet for educational purposes under the direct supervision of a teacher. Initial_______ 3. Photographs: Unless requested by me in writing apart from this contract, NTBCS has permission to use photographs/video of my child as a student in school newspapers, yearbook publications, and on the NTBCS website as appropriate and common for school communication and publicity. Parents understand that NTBCS is not responsible for students who are identified in newspapers, on websites, or at public events. Initial _______ 4. Handbook: I/we, the parent/guardian, agree to read and abide by school policies as stated in the parent/student handbook. Failure to do so may result in dismissal. Initial _______ 5. Early Withdrawal Penalty: Your financial obligation to the school is for the full annual tuition and all other fees. NTBCS prepares its annual operating budget based on the number of students registered to attend classes. Our commitments to pay salaries and other financial obligations continue even when students withdraw during the school year. NTBCS recognizes the fact that situations occur and students do withdraw from time to time. NTBCS believes in fairness and equity and will use the schedule below to determine your balance of tuition due based on when your student is withdrawn from classes. All parents at time of registration agree to the withdrawal terms as below. Parents on monthly payment plan that withdraw a student(s) during either semester agree to pay: a) The full tuition due up to the withdrawal date, and b) An amount equal to 25% of the remaining tuition due through the end of the semester. Parents who paid tuition in full at the beginning of the school year will be refunded 75% of the pre-paid tuition balance. NTBCS calculates the pre-paid tuition balance based on the number of full months from the withdrawal date through the end of the school year. Registration Fees are non-refundable if your student attends classes. a) You must be current on your tuition for your child to attend classes. b) No student records are release with open tuition balances. c) NTBCS respectfully requests you participate in an exit interview in the event you withdraw your student from classes. 6. Lost Library Book Fee: $20 or price of replacing the book (if book is of higher value). Student can replace the book themselves at no cost. Payment Policies and Procedures: I/we, the parent/guardian(s), have read and agree to abide by the stipulations as set forth in the NTBCS Financial Policies and Tuition and Fees document. If tuition is past due, any fees for field trips, class trips, etc. regardless of payers intent, will be transferred to the tuition account to satisfy any past due amount. Final grades, credits, and/or diplomas will be held until accounts are paid in full. Initial _______ 8. When differences of opinion exist, we will go first to the person with whom we have the problem. If not resolved, we will go with the individual to his/her supervisor. (Matthew 18:15-17) If any time during the training of our child, we can no longer work together in a spirit of unity, and all reasonable avenues of communication are exhausted, we will withdraw our child from New Testament Baptist Christian School.

Effective Date of Contract: This contract shall be effective only upon my/our signatures, receipt of NTBCS Enrollment Contract, acceptance by the school, and is subject to the admission policies of NTBCS. Enrollment as specified in this agreement may be cancelled pursuant to item #5 above by parent(s) or guardian(s) in writing. Application Fee (Due June 1st if late add $40.00) $75 (per family). Application Fee is required for those applying for attending New Testament Baptist Christian School. This fee covers the application processing, testing materials and billing set-up and must accompany the completed application form. This fee is non-refundable. This fee is per student and is due upon acceptance or re-enrollment and is NON-REFUNDABLE. This fee covers the rental of textbooks, consumable work books, achievement testing materials, and other miscellaneous classroom materials. This fee covers technology costs, medical supplies, and one yearbook per student. Initial _______ ABeka Book Kit (Due June 30th) $250 (per student). This fee is per student and is due by June 30th or at the time of registration for later enrollees and is NON-REFUNDABLE. This fee covers the cost of all the materials the student needs for the ABeka courses (the text books and workbooks). Annual Tuition NTBCS charges an annual tuition, which is due in full by June 30, 2012, or upon enrollment if enrolling past June 30th. This contract obligates you to this commitment regardless of absence, withdrawal, or dismissal. NTBCS does offer a deferred payment plan; details and associated charges are discussed below. Payment Methods Payments can be made by cash, check, and automatic bill pay directly to the school. Students Enrolling After September NTBCS prorates tuition for students entering after the first month of school. A student will be charged tuition equal to 1/10th of the total tuition for each month or partial month they attend NTBCS. All fees must be paid in full at the time of enrollment, along with the first tuition payment. All tuition payments must be completed by May. Curriculum Changes All curricula and programs are reviewed and evaluated at the end of the school year and are subject to change. The administration of New Testament Baptist Christian School maintains the right to make these changes as it sees fit for the benefit of the students; such changes do not constitute grounds for termination of this contract by enrollees. Accordingly, refunds will only be issued in accordance with the policy described above. This contract is subject to the laws of the state of North Dakota Should any part of this contract be determined to be unenforceable, this will in no way affect the remaining terms, which shall remain in effect and shall be binding on all parties. Tuition Discounts and Aid (Discounts will be not be applied if any outstanding amounts are due NTBCS) Full-time faculty and staff are entitled to a 30% tuition discount. Part-time faculty and staff (working 20 30 hours/week) are to receive a 20% tuition discount. Approved full-time Pastors are entitled to a tuition free education.

Statement of Cooperation

It is my (our) understanding that the policy for the school is to make no refunds on registration fees. I (We) also agree to meet all school financial obligations punctually. I (We) give New Testament Baptist Christian School, Kindergarten through 12th grade, permission for my child to take part in all school activities, including bus trips, sports activities, and school-sponsored trips away from the school premises. I further agree to hold the school and its agent harmless for the liability to my child or any guardian or parent thereof because of any claims on behalf of my child against the school or any agent thereof because of any injury or alleged injury to my child. Should legal action, for any reason, be taken against New Testament Baptist Christian School or any employee or agent thereof, on my childs behalf and the school or its agent not be found at fault, I agree to pay any attorney fees, court fees, damages or other costs that New Testament Baptist Christian School or its agent should incur to defend against such action. I (We) will cooperate with the teacher to ensure a safe and healthy environment. I (We) believe that discipline is necessary for the welfare of each student, as well as for the entire school. I (We) give permission for my childs teacher and/or other agent of the school to make and enforce classroom regulations in a manner consistent with Christian principles and discipline as set forth in the Scripture. The teacher and administration are given full discretion in the discipline of our child/children. This would include the issuing of detention, Saturday detention, suspension and expelling from the school program. Detention may be used after school for various offenses. Transportation will be fully the parents responsibility. The school administration reserves the right to dismiss, expel, or suspend any student at any time for any reason it may deem necessary. If a student is dismissed for any reason, the tuition will be prorated for the period of time that the child is in school. All new students are admitted on six (6) weeks probation. This Statement of Cooperation will be in effect for as long as my child/children listed (or others to be enrolled) attend New Testament Baptist Christian School whether it be in the Kindergarten, elementary, junior-senior high, or summer school. I (We) understand that should my marital status change that it is my (our) responsibility to have a corrected Statement of Cooperation signed and updated and delivered to New Testament Baptist Christian School. Attending New Testament Baptist Christian School is a privilege. Student Guidelines

Let no man despise thy youth; but be thou an example of the believers, in word, in conversation, in charity, in spirit, in faith, in purity. (I Timothy 4:12)
Welcome to New Testament Baptist Christian School. Our purpose at NTBCS is to educate in the light of Gods Word: to help students grow spiritually, academically, mentally and physically. This requires much planning, organization and work on the part of the student. Accepting and fulfilling the standards of conduct will result in establishing personal growth and appreciation of others. These attitudes will promote true school spirit. Proper conduct is emphasized and expected at all times from our students and is an essential element to learning. Improper conduct will be dealt with justly. If a warning does not correct the behavior, then, further action must follow. Parents will receive direct communication by letter, phone call, or conference to determine course and direction. Correction will be cared for in the office and will be based upon the severity and repetition of the offense. Correction may include: additional assignments, office referrals, loss of privileges, special projects, probation, suspension, and ultimately expulsion. In the final analysis, the administration will determine the appropriate measures to be taken. Parents who at any time disapprove of these measures should remove their children from NTBCS. Students may be withdrawn any time the administration deems the students presence in the school to be detrimental to the spiritual, mental, or physical welfare of any other student. Examples of inappropriate conduct include: disobedience, disrespectfulness, failure to do homework, dress code violation, fighting, name calling, profanity, immorality, cheating, stealing, destruction of school property, rock music, profane or immoral TV/videos, drug abuse, gambling, use of tobacco or alcohol, attendance to movie theaters, dancing, or any other action unbefitting a student attending a Christian school (i.e. inappropriate behavior with the opposite sex). NTBCS has a uniform standard and it is based on two criteria: Bringing honor and glory to God following the Biblical standards of modesty (I Timothy 2:9 & Romans 12:1) and the students personal safety. Dress standards for school trips or school sponsored activities will be discussed at orientation. It should be understood that as a student of New Testament Baptist Christian School, you are a representative of our school and that your behavior at all times should not violate the standards of the school. Parent School Guidelines

To maintain harmony with the Christian home, church, and school, as a parent you are asked to: Attend the back-to-school orientation meeting to be held at the beginning of the school year. Attend other parent meetings and conferences scheduled by the school. Make it your first priority to attend all school activities that your child is involved in; thereby assuring him/her of your loving support (e.g., programs, athletic competitions, etc.). Attend your local church regularly, realizing that NTBCS is a supplement not a substitute for a Godly home and family. Regular church attendance is Biblical (Hebrews 10:23 25; I Thessalonians 5:11). Support the school with your prayers and volunteer service in assisting with fund-raising and all other school related activities. Your prayer support of our school is absolutely essential. This is a spiritual battleground, and the battle must be fought with spiritual weapons. (I Corinthians 10:5 11; Ephesians 6:12; I Timothy 2:1 4). Pay your tuition on time and are due UPON RECEIPT of statements which are mailed at the beginning of each month, July through April. Payments MUST be received in the School Office by 3:00 PM on the 20th of the month to avoid a late fee. Checks dated the 20th or 31st, or postmarked the 20th or 31st, but not received by the 20th or 31st, will be considered LATE. NTBCS is not responsible if checks are sent with students and not turned into the School Office on or before the 20 th or 31st of the month. Support the school in all matters of discipline involving your student. Understand that your student will be dismissed from school for any incident involving the use of alcohol, tobacco, drugs, weapons, or any instance of immoral or unseemly behavior/speech on or off campus at any time. When you have a problem or concern, please contact the person (teacher first, helper, etc.) most directly involved. If it is not resolved, then contact the Principal for clarification. If at all possible leave school related issues at school. Try not to talk to the teacher or the principal about school issues at church. Contact the school during school hours. We expect you to agree not to share problems with those not listed in the above steps. (Matthew 5:23 24, 18:15 16; James 3:5, 9 10). Refrain from taking your student out of school unnecessarily. Regular attendance is important. The faculty is willing to help students with makeup work, but you must take the initiative. Advance notice is required. Contact the Principal or office directly to let us know your reasons for withdrawal. We appreciate your input and insight. If you are moving or financial needs arise, we appreciate knowing how we may assist you. Support NTBCSs Mission, Purpose, Goals, Philosophy, Doctrinal Statement, rules, and policies, understanding that while you may not always agree with a rule or policy per se, your support is still necessary and will be honored by God. (Romans 13:1 4; Hebrews 13:17; Deuteronomy 6; Galatians 4:1 2; Hebrews 12:9 10). NTBCS must have your heart felt support, especially from fathers, if we are to have an effective ministry with your child. You must waive any right to litigation regarding any dispute with New Testament Baptist Christian School and/or New Testament Baptist Church and agree to accept the Schools decisions as final in all such matters.

NEW TESTAMENT BAPTIST CHRISTIAN SCHOOL July 1, 2012 through July 1, 2013 FIELD TRIP RELEASE AND WAVIER OF LIABILITY (AGREEMENT) The undersigned, in consideration of my child being permitted to participate in various field trips, and excursions (hereinafter referred to as TRIPS) sponsored by or through NEW TESTAMENT BAPTIST CHRISTIAN SCHOOL (hereinafter referred to as SCHOOL) and/or NEW TESTAMENT BAPTIST CHURCH (hereinafter referred to as CHURCH), I, for myself, my personal representatives, assigns, heirs, and next of kin: 1. ACKNOWLEDGE AND UNDERSTAND that, although precautions will be taken to safeguard participants from any accident or harm, there are certain risks inherent in any form of travel to and from any event or activity sponsored by or associated with the CHURCH and/or SCHOOL. Knowing that, I further acknowledge that travel to and from any such activity will be conducted over public roads and facilities open to the public and upon which the hazards of traveling are to be expected. 2. I fully accept and assume all such risks and responsibility for any losses, costs, and damages which I may incur as a result of my childs participation in any travel to and from any event or activity that may involve the CHURCH and/or SCHOOL, and/or by other forms of transportation, whether private and/or public. 3. COMPUTER USER AGREEMENT


Every student and parent must sign the Computer User Agreement before the first day of school and submit to the Administration Office.

For Grades 6-12: REASONS FOR THIS POLICY: New Testament Baptist Christian School (NTBCS) is providing computer network and Internet access for its students and employees. This service allows employees and students to share information, learn new concepts, and research diverse subjects. NTBCS has adopted this Acceptable Use Policy (Policy) to set guidelines for accessing the computer network or the Internet service provided by NTBCS. Every year, students who want computer network and Internet access for that upcoming school year need to sign and submit a copy of the Policy to NTBCS. Students also must have their parents or guardians sign this Policy. By signing this agreement, the student and parent or guardian agree to follow the rules set forth in this Policy and to report any misuse of the computer network or the Internet to a teacher or supervisor. Parties agreeing to this policy also understand that NTBCS may revise the Acceptable Use Policy, as it deems necessary. NTBCS will provide notice of any changes either by posting such a revised version of the Policy on its website or by providing written notice to the students and parents or guardians. INTERNET USES: Use of the electronic information resources in the school shall be to improve and support the educational process by providing access to global information and improving communication between students, employees of New Testament Baptist Christian School, parents, and community members. ACCEPTABLE USES: All Internet or computer equipment use shall be consistent with the purposes and goals of NTBCS. It is imperative that users of the Internet or computer equipment conduct themselves in a responsible, ethical, moral, and polite manner. All users must abide by all local, state, and federal laws. The Internet user accepts the responsibility of adhering to high standards of conduct and the terms and condition set forth in all parts of this policy. IMPERMISSIBLE INTERNET & COMPUTER EQUIPMENT USES: computer equipment are prohibited: The following uses of the Internet &

1. Any violation of applicable school policy or public law by such use; 2. Any activity that is immoral or contrary to the high moral standards which must be maintained in an educational setting; 3. Any attempt to bypass school security is forbidden; 4. Accessing or transmitting of immoral, obscene, pornographic, profane, lewd, vulgar, rude, defaming, harassing, threatening, disrespectful, or otherwise inappropriate images or information, or receiving such information from others; 5. Any commercial use, product advertisement, display of personal information, or promotion of political candidates; 6. Any violation of copyright, trade secret, or trademark laws; 7. Any attempt to damage, disrupt, or interfere with the use of any computer or electronic information resource; 8. Any attempt to access information beyond the users authorized access to any electronic information resource; 9. Any destruction, defacement, theft, or altering of school equipment; 10. Any storing of illegal, inappropriate, or obscene material on school owned electronic equipment.

PRIVILEGE: The use of the Internet and computer equipment within NTBCS is a privilege. It is confidential information to the user unless it is transmitted to others with the users permission. Violation of this policy can result in the loss of computer access privileges or further disciplinary action. MONITORING: NTBCS reserves the right to monitor and review any material on any machine at any time in order for the school to determine any inappropriate use of computer resources.

DISCLAIMER OF ALL WARRANTIES: NTBCS makes no warranties of any kind, whether expressed or implied, for the services provided in connection with use of the Internet or computer equipment. Neither NTBCS nor any supporting Internet services will be responsible for any damages that an Internet user suffers. NTBCS expressly disclaims any liability in connection with the loss of data resulting from delays, non-deliveries, failure to deliver, mistaken deliveries, viruses, backup device failure, service interruptions, or other unforeseen reasons caused by NTBCS or the Internet server or by the users error or omissions. Use of any information obtained via the Internet is at the users own risk. NTBCS expressly denies any responsibility for the accuracy or quality of information obtained through any Internet service. All users must consider the source of any information they obtain and evaluate the validity of that information. SECURITY: NTBCS will implement security procedures on Internet access to protect against unacceptable use. Computer users are responsible for the security of their files and passwords. Sharing of any usernames or passwords to anyone is not permissible and may result in the loss of account privileges. Computer users will be held accountable for any activity under their account. Any security problems should be reported immediately to the school/site administrator. ENCOUNTER OF CONTROVERSIAL MATERIAL: Internet users may encounter material that is controversial which the user or administrator may consider inappropriate or offensive. The school has taken precautions to restrict access to inappropriate materials through a filtering and monitoring system. However, it is impossible on a global Internet to control access to all data that a user may discover. It is the users responsibility not to initiate access to such material. Any site or material that is deemed controversial should be reported immediately to the appropriate administrator. NTBCS expressly disclaims any obligation to discover all violations of inappropriate Internet access. TERMS OF USE: 1. Only registered students of NTBCS qualify for computer use under this policy. 2. Only the authorized users who have signed the user agreement shall have computer access. Users are ultimately responsible for all activity while using the Internet. 3. The headmaster may suspend or terminate any computer users access for any reason. 4. All Internet or computer equipment access of a student is automatically terminated at the moment of withdrawal, graduation, or expulsion of the student from NTBCS. PENALTIES FOR IMPROPER USE: Any user violating this policy or applicable state and federal laws may be restricted from Internet use or use of any or all computers (in addition to other disciplinary options). Such additional options may include suspension, expulsion, and/or referral to legal authorities. STUDENT: I understand and will abide by the New Testament Christian School Student Computer Acceptable Use Policy. I agree that I will keep my unique login and password information private and not allow others to have access using my account.
