The appropriate National Account Number should appear on all Headers. If you install a location with 100 wearers or more, you must verify that the DC has inventory to cover your installation before an install date is given to the customer. This includes all Service Express and Non-Service Express items. Supply Chain Management cannot guarantee sufficient inventory on a large install without prior warning. RATE ADJUSTMENTS: Rates for services are guaranteed a minimum of 12 months from the effective date of the local service agreement. This account is not rate managed but it is being reported for sales reporting purposes. Please report your customer numbers to National Accounts under N92881CHV (rental) or N92881-CHVL (lease).
