Mass Bay Junior Olympic Festival 2012 Annisquam Yacht Club Minor Waiver, Medical Consent, and Media

By allowing my minor child to participate in this event I agree to the following policies. Minor Waiver of Liability I hereby acknowledge that serious accidents occasionally occur during sailing activities and that mortal or serious personal injuries and/or property damage or other loss may result from participation in the Regatta. I understand that participation in this Regatta is voluntary and that my child incurs risk by participating, including the possibility of injury or death. In consideration of the acceptance of my childs entry, I waive, for myself and for my child, any and all claims, charges, losses, and liabilities, including those caused by negligence, against the Annisquam Yacht Club and its officers, governors, contractors, employees, members, agents, sponsors, and volunteers, that may arise from or in any way be in connection with the activities of the Mass Bay Junior Olympic Sailing Festival, and agree not to sue or to bring any claims of any nature whatsoever against the host organization(s) or any of the persons and officers, named or unnamed, above, who may be acting on the hosts behalf. I understand that I am responsible for the actions of my minor child while he or she is participating in the regatta both on land and at sea and that I am solely responsible for whether or not my child participates or sails each day. Consent to Treatment (minor) In the event of accident, injury, or illness to myself or my minor child while in, on or about the premises of the Annisquam Yacht Club or while participating in any activity sponsored by or under the auspices of the Annisquam Yacht Club or the Mass Bay Junior Olympics, under circumstances where I am physically unable to consent or am not present: 1. 2. 3. I hereby voluntarily consent to the furnishing of such medical care and treatment by any hospital, or physicians as such hospital or physicians may deem necessary or advisable. I authorize any officer, member or volunteer of the Annisquam Yacht Club to consent to such medical care or treatment. I agree to pay all costs of such medical care or treatment and to hold free and harmless of and from any and all liability for such cost the Annisquam Yacht Club and its governors, members, and volunteers.

Media Release I grant to Annisquam Yacht Club/Mass Bay Junior Olympics 2012 and its sponsors, its representatives and employees the right to make, use and show, from time to time at their discretion, any photographs, motion pictures and live, taped or filmed television and other reproductions of him or her during and after the period of the competition without compensation. I authorize Annisquam Yacht Club, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Annisquam Yacht Club may use such photographs of me and/or my daughter/son with or without my name and for any lawful purpose, including, for example, such purposes as publicity, illustration, advertising, and web content.
