On 09-09-10, at 0809, I was dispatched to 6488 Sandlers Preserve Dr. in reference to a missing person.

Upon my arrival at 0813, I was flagged down by an unknown white female who stated that a suaplcious person
was laying in tho park In the same block as the listed residence.
I made contact with the subject, a black female, which matched the description of the missing person.
Investigation revealed that this subjeet was the victim.
The victim had what appeared to be a self Inflicted gun shot wound.
Rescue and Engine 31 responded to the scene.
fhe victim was transported via l-ife Flight to Shands.
Sgt. J. Stacy #5675 responded to tho scene.
Lt. R. H.Eikins #5858 responded to the scene.
Homicide was notified and responded to the scene.
The victim was Baker Acted.
An E.T. was assigned by H.Q.
Homicide will follow-up.

This 1$ the attempted suicide investigation of Shauna M. Terrell, (black female, 13 years old) which occurred on
()9.09-2010, at 6488 Sandlers Presorve drive, in Jacksonville, Duval County, Florida (Sub-sector L-1). The victim

The incident occurred at a Gazebo which is located at the 6400 block of Sandlers Preserve Drive. The Gazebo is
located on the west side of Sandlers .,reserve Drive. The Gazebo is red in color.

BEGINNING OF INVESTIGATION
Thursday/0909-2010/0900: Detective Hougland and I were dispatched to 6488 Sandlers Preserve Drive, in
reference to an attempted suicide investigation. It should be noted that Detective Hougland wu diverted to
Shands Hospital to check on the status ofthe victim.
Thursday/09-09-2010/0950: I arrived at 6488 Sandler'S Preserve drive. I met with Officer S.C. Crawford #7743,
who advised me that he was dispatched to 6488 Sandlers Preserve Drive, in reference to a missing person.
Officer Crawford statod that upon arrival, he was flagged down by an unknown white female, who stated that
she saw a suspicious person lying in the park, close to the address where he was dispatched to. Officer
Crawford stated that he went to check on the individual and noticed that she matched the description of the
missing person. He said that the victim had what appeared to be a gunshot wound to the left side of her cheat.
Officer Crawford stated that Re.cue #31 responded to the scene and the victim was transported to Shands
Hospital via helicopter. He stated that there was a witness waiting for me in front of the victim's house, who
stated that the victim called her to say goodbye, because she was leaving and not coming back.
Thursday/09-09-201 0/1000: I met with Evidence Technician H.R. Brooks #7536 and I requested that she
photograph the victim's residence and where the victim was discovered.

Thursday/09-092010/1015: I met with Mrs. Whitlow (witness}, who stated that the victim called her to say
goodbye. She said that the victim said that she was leaving and would not tell her where she was going. She
stated that she told the victim several times to put her mother on the phone, but she reftlsed to get her mother,
because she was leaving and not coming back. She stated that the victim hung up the phone, she attempted to
call her back, but the line had been disconnected.
Mrs. Whitlow said that she drove to the victim's house to check on her, but her mother did not know where the
victim was. She said that the victim's mother called 911 and advised them that her daughter was missing. She
stated that the victim's mother told her that the victim had written several disturbing letters and they were on
top of her bec:l.
Thursday/09-09-2010/1025: I called Detective Hougland and asked him to get the parents of the victim, to sign a
Consent to Search Form.
Thursday/OQ..09-201 0/1035: Detective Hougland called me and stated that the Consent to Search Form was
signed by the victim's parents.
Thursday/09-09-2010/1040: I entered the victim's residence with Evidence Technician Brooks and I asked her to
take overall pictures of the residence, the victim's bedroom and collect the letters from the victim's bedroom.

Thursday/09092010/1145: I talked to Mr. Terrell (victim's Father) and he stated that the victim did not know the
combination to the safe, which Is located in his bedroom closet.
Frlday/09-10..2010/1400: I met with the victim's Pilre.nts at Shands Hospital. They advised me that their daughter
Is doing better. They stated that the victim had attempted suicide two times in the past by overdosing in pills
and they were going to admit her to a Hospital after she recuperates from her injuries.

Monday/09-13-2010/0925: I talked to Mrs. Terrell, who advised me that her daughter was doing well at the
Hospital and It looks like she is going to recover from her Injuries.
