Internship at Cheeny Kum!

I visited all three branches of Cheeny Kum clinic in thane. Ashwini maam introduced me as to what Cheeny kum is, how does it function, using its website, registering a patient, message logs etc. The first most attribute that she made me realize is Communication. Clear, simple, kind language used while conversing with patients and other professionals while I was calling them for either renewal or to ask whether how was the diet regime going on. One has to be good with technology, so she showed me how to use fat composition analyzer, EZ scan , glucometer. Also, how to read their respective reports and come to appropriate solutions. During the period I was there, I met three patients. 2 were new patients while 1 was a follow up case.

The follow up case was a female with diabetes whose glucose readings were made into chart format for evaluation. This format of glucose reading chart helped the patient to understand his/her condition easier. She was although doing fine overall, little regularity was needed in her case. One of the new patient came to collect her EZ scan report of the previous health camp held. She wasnt diagnosed with any kind of disorder. But she was advised to do her fat composition as she was close to becoming a pre-diabetic and was over weight as well. Her fat % came high, thus, was advised to lose fat. I learnt how to take a complete dietary recall including other concerned and important questions such as regularity of menstrual cycle, thyroid problems etc. The other client wanted to get his EZ scan done as somebody referred him to do it . Therefore I was taught how to use the machine. He was diagnosed as pre diabetic with 9697% insulin resistance. His dietary recall was taken and being an software engineer his work style was sedentary and also that he was a South Indian, his meal patterns and food taken indicated a poor lifestyle. Along with the ample knowledge gained in three days, one more distinct characteristics she taught me was Business development. Ways to broaden horizons with innovative ideas of health programs for different age groups with different firms and classes of individuals. (Mahesh tutorials, Corporate plans)

Golds Gym
The branches of Golds Gym where I interned are: Nepeansea road India Bulls Kandivali (W) Kandivali (E) I saw how the In Body is done. Dietary recalls and counseling techniques. The trainer explained me about the various machines used for various reasons and the muscle used or trained during that particular activity. There was a beverage recipe contest among members of the gym which was a fun event.

Coffee with Cheeny Kum: Listed down the diet food products available at a popular store with details such as ingredients, method of using oil (whether sprinkled or oil removing technique used) and their cost for the amount. Prepared a FAQs list, listed down the strategies or the things to be worked upon for the preparation of the event. Called patients to attend the event, asked for their confirmations. Worked on the quiz for the event.

Case studies
Case study 1: 25/M/Cricketer, he is a Ranji trophy player. He has a past history of kidney
stones and since birth, he has only 1 kidney. Plan a diet for him concentrating on his protein intake. As he is a cricketer post recovery is the most important part of the sport. Also, he needs to lose weight. Practice session: 5:30 to 8:00 and 4:00 to 6:00pm.
Points considered while planning diet were: Limit his protein intake compared to other cricketers. Avoid all foods rich in oxalate so that reoccurrence of stones are unlikely. Ensure that enough water goes in the diet especially during work outs to prevent dehydration and limiting the chances of stones again. At least, 2-3.5 L of fluids in the entire day The list of foods high in oxalate and low in oxalate is mentioned in the dietary guidelines so that he knows which foods can be had in good amounts and which should be restricted.

Case study 2:
50 yr/f, is a diabetic on OHA and has undergone CABG. 2nd day after surgery. Plan a liquid to soft diet.

Points considered while planning a diet: As dysphagia is commonly experienced by patients undergone CABG, a liquid to soft diet is recommended. Start with liquid and gradually progress to semi- solid i.e. soft diet. Functional foods like green tea, barley are incorporated in the diet. Avoid spicy, coarse and hard textured solid foods initially. As the patient recovers and can have normal diet, include more of heart healthy foods and also consider its glycemic index.

Case study 3: Geriatric with IBS and gluten intolerance.

Point considered while planning a diet: Avoid foods which would further aggravate the problem. Gluten intolerance could be the reason of IBS, therefore foods containing the protein gluten should be restricted. Wheat and wheat products should be monitored in the diet, along with barley, rye and oats. Include products of rice and its flour in combination with pulse flour.

Diet evaluation:

Three exchanges of fruit is practically not possible in a day and cannot be followed for everyday. Instead of 6 almonds, 4 almonds and 2 walnuts can be given. 3-3 exchanges of milk and milk products in the entire day is a good choice, although should be advised to use low fat milk or toned milk as paneer and buttermilk would have high amount of fat content. Exchange of cereals in the entire day is 2 which is very low. Breakfast is not balanced. Dinner: It is not possible to have 1 k. vegetable + 1 k.dal +3 pcs of paneer without a staple on a regular basis. Details in the diet like type of milk, bread, which vegetables should dominate in the sandwich, soup(whether a vegetable soup or cream soup), salad with the dressing or without it, if dressing then which is preferrable. Therefore this diet wouldnt be followed for a long time.
