Adenocarcinoma 95 %
Antro gstrico continua siendo la localizacin ms frecuente
caractersticas distintas:
Cncer gstrico intestinal Predomina en personas de edad avanzada. Sexo masculino. Frecuente en zonas de alto riesgo (epidmico).
Cncer gstrico difuso Predomina en personas jvenes. Sexo masculino = sexo femenino. Tendencia gentica Frecuente en zonas de bajo riesgo (endmico).
Epidemiologa
Se producen: 640,000 muertes en el mundo por ca gstrico. Equivalen al 15 % de muertes por ca.
Segunda neoplasia ms comn del tracto digestivo. Japn presenta 82 casos por cada 100 000 habitantes. Mxico tiene 3.3 casos por cada 100 000 habitantes.

Etiologa
Factores genticos y carcinognicos determinan la transformacin del epitelio gstrico normal en neoplsico. El medio ambiente juega un rol importante: Dietas ricas en caloras, carnes rojas, comidas saladas con altos contenidos de nitritos y nitratos. Comidas ahumadas. Comidas con preservantes, carencia de frutas y verduras frescas.
Factores de riesgo: Helycobacter pylori. Historia familiar de cncer gstrico. Tabaquismo. Anemia perniciosa. Grupo sanguneo A. Gastrectomas previas. Virus de Epstein Barr.
Signos y Sntomas
Prdida de peso. Disfagia. Distensin abdominal. Nuseas vomitos Hematemesis Dolor abdominal Eructos excesivos Halitosis Meteorismo Prdida involuntaria de peso Deterioro de la salud en general Llenura precoz
En las fases tempranas los sntomas son leves, vagos e inespecficos: indigestin, epigastralgias etc.
Evaluacin Operatoria Premisa: extirpar totalmente el foco primario y sus conexiones linfticas. Cirujano analizar: tumor y sus vas de diseminacin para determinar pronostico y tratamiento quirrgico. Localizacin Compromiso ganglionar Grado de invasin
LOCALIZACIN Localizar y delimitar la lesin en sentido vertical y transversal. Al variar la localizacin vara el drenaje linftico y por lo tanto el tipo de reseccin.
