PLEASE BE AWARE THAT ALL ACCIDENTS, INCIDENTS AND NEAR MISSES MUST BE REPORTED AS PER CTSD HSE PROCEDURE 7000-E-00-10-S-0612-01

ACCIDENT/INCIDENT INVESTIGATION

Definitions. Accident:- An unplanned unwanted occurrence that disrupts progress and results in injury, damage or loss of production.

Any accident or incident and even near miss must be investigated with the sole purpose of Determining the exact cause Making recommendations to prevent recurrence

It is vital that any incident is investigated

Immediately By the right person(s) With the right equipment And the authority to do it

ACCIDENT/INCIDENT INVESTIGATION WHO ARE THE RIGHT PEOPLE? HSE Personnel Management Engineering Police Medical staff Any others as dictated by Management

WHO TO INVOLVE AND WHEN? HSE Immediately for all incidents In the case of minor HSE may be the only investigator. He/She will have the authority to involve others they require. E.g. Doctor. Usually HSE have all the necessary equipment at hand.

ACCIDENT/INCIDENT INVESTIGATION WHO TO INVOLVE AND WHEN? MEDICAL STAFF If requested by Management In event of death or serious injury In cases of suspected criminal act involving medicinal substances

ACCIDENT/INCIDENT INVESTIGATION WHO TO INVOLVE AND WHEN? ENGINEERING At the request of HSE/Management Where process or engineering is involved Where such advice is necessary

ACCIDENT/INCIDENT INVESTIGATION
WHEN TO BEGIN THE INVESTIGATION. As soon as you are informed of the event. WHERE TO START. Try and start with the SCENE. Then start your interviewing of those involved, witnesses etc.

ACCIDENT/INCIDENT INVESTIGATION BUT BEFORE YOU DO ANYTHING YOU MUST CLEAR YOUR MIND OF ANY PRECONCEIVED IDEAS. YOU MUST GO WITH AN OPEN MIND ALSO REMEMBER THE PURPOSE OF YOUR INVESTIGATION IS NOT TO APPORTION BLAME OF ANY KIND.

Camera is valuable
Notepad and pens Measuring tape Flag tape

ACCIDENT/INCIDENT INVESTIGATION
WHAT DO I NEED TO LOOK FOR (DO NOT DISTURB ANYTHING IF AT ALL POSSIBLE!!!!!) Evidence of any kind. Damage, equipment involved, position of equipment and tools etc. Position of any injured person prior to and AFTER the accident Spillages, stains, clothing.

WHAT DO I NEED TO LOOK FOR (contd.)

Time of day/night Wind conditions Other weather conditions, rain snow etc. Any permits to work Any other jobs going on in the vicinity Witnesses!!!!!!

ACCIDENT/INCIDENT INVESTIGATION WHERE DO I GO NEXT? Check on any injured person (s) but before you do make sure that the doctor confirms he/she is fit to receive you. Try and get information from them but do remember they are HURT. Make sure you have a witness when you speak to them, preferably medical.

ACCIDENT/INCIDENT INVESTIGATION WHERE DO I GO NEXT? (contd) Get a statement from them if possible and get them to sign it, again if possible. After talking to the injured person now talk to the doctor, his/her information will be very valuable.

ACCIDENT/INCIDENT INVESTIGATION NOW YOU CAN TALK TO ANY OTHER WITNESSES. Although we must never apportion blame It is certain that something or someone caused the accident. Witnesses may well be frightened and/or guilty. You must be able to put witnesses at ease but at the same time get all you can from them. EVEN IF IT IS LIES!!!!!!!

INTERVIEWING WITNESSES DO s. Try and segregate witnesses but do it subtly to avoid fear. If you do not segregate them, they can swap stories and establish a common approach. Assure them of your intentions i.e. ONLY to establish cause. Ask them if you can take notes. Let them see what you are writing

INTERVIEWING WITNESSES DO s. (contd) Smile, do not frown. Ask them to make a written statement. Thank them for their cooperation, even if they have not been cooperative. They may have been lying through their teeth but next time???

INTERVIEWING WITNESSES DON Ts Do not sit behind your desk, get out and sit with them. Do not interrupt any talking they do. Do not take notes while they are talking. Do not use leading questions

ACCIDENT/INCIDENT INVESTIGATION SUMMARISING Take all your statements Link them with the photographs and notes. Link these with medical, engineering reports Discuss your findings with any senior HSE or management. If you are not satisfied refer the investigation to a higher authority

ACCIDENT/INCIDENT INVESTIGATION NOW YOU WILL INVESTIGATE AN ACTUAL ACCIDENT. REMEMBER WHAT YOU HAVE BEEN TOLD, MAYBE YOU WIL GET ONLY VERY BRIEF DETAILS TO START WITH.

YOU ARE IN YOUR OFFICE WHEN YOU GET A CALL THAT A MAN HAS BEEN INJURED AND TAKEN TO THE SITE CLINIC. NOW GO AND INVESTIGATE THIS ACCIDENT

FOR THE PURPOSES OF THIS EXERCISE I AM EVERYONE YOU WANT TO TALK TO I AM THE INJURED MAN, THE DOCTOR, THE WITNESSES, THE MANAGER, THE DOG THE CAT AND THE MOON!
