NOTE: Vertical dimension has to be reevaluated, and temporary prostheses are realized for testing the biomechanical requirements of this large restoration. During this period, it is necessary to restore esthetics and function of anterior teeth. However, functional tests according to phonation, anterior guide, have to be performed. These restorations will be modified during this period, which is easier with a resin composite material. EsthetX restorative is chosen for the large panel of shades and opacities available.
The clinical situation of this patient, 55 years old, is dramatic, with bruxism, attrition, and absence of posterior teeth.

The cervical shade is Regular Body A3.5. The polymerization of a "cervical ring" of composites is achieved because polymerization concentration is of interest in this special situation. Translucent Enamel Shade YE is used for the incisal margin. However, some ridges of Regular Body Shade A2 contribute to a natural irregular translucency.

Contouring, polishing, and finishing are performed with the Enhance System and Prisma Gloss Polishing Paste. Abrasive strips are also required for the proximal surfaces. The final result in occlusion will be adjusted after conducting the same protocol for lower incisors.

EsthetX B1 Regular Body Shade is placed on the left lateral incisor and curedwithout etching or bonding. This is done to check the shade match of cured composite to the natural tooth color. NOTE: At that time, both teeth were restored. From a professional point of view, the marginal integrity of both restorations was unsatisfactory, but the major problem was an unsatisfactory color and even more important transparency match between the tooth structure and the restorative material. Both teeth reacted to vitality testing.

NOTE: A long beveling is often used to achieve a smoother change in color, but with conventional one shade restorations this is very often intended to mask the fact that the overall color match and even more important, transparency match is unsatisfactory. Using a core buildup with darker and less transparent shades of EsthetX, a shorter bevel still leads to an excellent color match.
After the removal of the pulpal seal, there was still a larger area of exposed soft tissue. It was decided to renew the seal. A short bevel was prepared in the marginal areas.

NOTE: As can be seen in the slide, this layer was contoured in a special way, modeling mamelons and embedding spots of a lighter shade in order to achieve a more lively appearance of the restoration.
The EsthetX restorative material was applied in three different layers. In the first step a core buildup was performed using the Opacious Dentin material. After light curing of this layer for 20 seconds, a second layer was applied using the Regular Body of the selected shade guide recipe.

NOTE: With this step it is very important not to dead flatten the surface, since this prevents the restoration from looking lively. During finishing, it was the intention to follow the contour of the remaining tooth structures, as well as to copy the surface contour of the adjacent teeth.
Contouring and prefinishing of the outer surface is easily achieved with a fine diamond finishing bur.
