INTRODUCTION
In order to start our consolidated survey, it is necessary to have a certain Barangay wherein we will be conducting surveys by way of interviewing each family regarding their health status, environmental sanitation and biographical datas from each family member. Barangay is considered as an urban area, as we start roam around the Barangay, we have observed that theyre not practicing how to properly dispose their wastes and garbages. Most people residing in that Barangay are informal settlers, the land is owned by the government, they have Daycare, Elementary and Secondary School & they have also a chapel within their Barangay. They dont have Barangay Health Center , theyre just depending on Lawaan I Health Center or RHU. And as we arrived there during our duty days, people were very hospitable, they tried to entertain and welcome us wearing smiles on their faces. They were able to understand and answer our interrogation processes, in other way, they were very cooperative and participative. In this matter, the goal of improving and enhancing the health status of each family and the community environment will be focused.

HISTORICAL BACKGROUND

Barangay Lawaan III was founded in 1969 and its name was taken from the tall and beautiful endemic Lawaan trees that were plentiful. It was said that when our forefathers came to cebu, this place was full of vegetation and its lush forest extended up to the area of Manipis. The red and white Lawaan species were in abundance.
Part of Lawaan was a place called Bacayan which was near the Mananga River. In fact, the Mananga Bridge in Tabunoc used to be called Taytayan sa Bacayan. It is likewise the home of various subdivision of the city. In the year 1981, Barangay Lawaan was divided into three Barangays namely: Lawaan I , Lawaan II & Lawaan III, thus this barangay was born. This barangay is now more accessible with the newly constructed hanging bridge that stretched towards Barangay Lagtang. Under Lawaan III was our assigned place Sitio Kasagingan. We asked the residents on that sitio regarding on what was really the Historical Background why their place came up into Kasagingan. They stated that the term Kasagingan originated from the word Saging, and then later came up into Kasagingan. Long time ago before the informal settlers came, that place has numerous banana planted on it. There were just few houses present surrounded with the plenty banana. They also said that our forefathers were the one who planted those bananas. As years gone by, people decided to cut those bananas and built some houses instead. But before that, they would role on their sitio, they decided to call their sitio Kasagingan which commemorates to those bananas present for the past years.

INTERPRETATION: Based on the data gathered, out of 260 populations, 129 are males which constitutes 47.62% and 131 are females which constitutes 52.38%

ANALYSIS:
Based on the data gathered, the number of females in the community is greater than the number of males. Women in their community are more active in finding productive and sustainable ways to meet their needs of everyday living including their food, transportation, education and etc. but there are some couples who help each other in doing their means of living, like in making rags. They work together and consider themselves as self-employed.

Out of 260 population 4% are infants (0-1) year-old, 6% are toddlers (2-3) years-old, 9% are pre-schoolers (4-6) years-old, 13% are school age (7-12) years-old, 11% are adolescents (13-17) years-old, 51% are adults (18-49) years-old and 6% are geriatrics (50 years & above).

ANALYSIS:

Based on the data gathered, there are 60 families a total of 260 members. Each family is approximately 3-5 members. Adult ranks as the highest number of population in sitio Kasagingan Lawaan III who are in the 18-49 years-old. The school age group ranks as the 2nd who ages from 7-12 years-old. They usually cope on outdoor activities and are usually prone to cough and colds. They should take vitamins for them to have the immunity against foreign substances. Adolescents rank as 3rd, pre-schoolers rank as 4th, geriatrics and toddler as 5th and lastly, infants, there are less infants in their community.

CIVIL STATUS
INTERPRETATION: Based on the survey, 185 (71%) of population are single which include the infants, preschoolers, toddlers, school age and to those who are not married yet, 4(1%) are separated, 26 (10%) are live-in, 38 (15%) are married and 7 (3%) are widowed.

ANALYSIS:
In this community, there are many single which include the children, infant, preschoolers, toddlers and etc. while others are married, some of them are live-in due to early of engaging intimate relationships and that they already build a family at such young ages. Since there are live-in partners in the community, and as we have observed, most of them appeared to have poor educational attainment. In small number, separated individuals are recognized and issue relating to such financial problems.

INTERPRETATION:

During the survey out of 60 families, 39 (65%) are aware about family planning but not all of them are practicing the family planning methods, and the remaining 21 (35%) are not aware and not practicing.

ANALYSIS:

Most of the families are aware and have attended seminars about family planning. But not all of them are applying those methods and that they have large family members. In this connection, they have knowledge on how to practice some of the Family Planning methods. According to them, Family Planning is very important because it really help them in controlling their family size, but there are also some families who are unaware about family planning for the reason that they dont have time to attend seminars and community activities which is related to family planning and health promotion for their own benefits. Nevertheless, those families who are unaware were asking informations about family planning to those families who have knowledge about it.

EDUCATIONAL ATTAINMENT
INTERPRETATION: Out of 260 people, 12 (14.12%) are Elementary Graduates, 102 (65.56%) are Undergraduates, 36 (42.35%) are High School Graduates, 51 (32.28%) are High School Undergraduatesm and 37 (43.53%) are not schooling. ANALYSIS: Based on the survey, most of the people in Sitio Kasagingan are Elementary Undergraduate, for the reason that they cannot afford to educate themselves or their parents due to low of income. There are some people who are High School Graduates and unfortunately High School Undergraduates. Because they just easily get involved into relationships and build a family of their own at such young ages. And so they stop schooling and had poor educational attainment. The families that we have interviewed said that they just prefer of not continuing the education level that they have started and just focus on their work to provide the needs of their children.

INTERPRETATION Based on the data gathered, out of 90 individuals excluding the infants, toddlers, preschoolers, and school age,39 (43%) are engaged to rag making, 20 (22%) are in driving, 10 (11%) are divulged to construction, 6 (7%) are in house keeping, 5 (6%) who are vendors and engaged in cosmetology, 3 (3%) are in furniture industries, 1 (1%) in tailoring, plumbing, care giving, and security agency. ANALYSIS Based on the data above, majority of the people in the area are rag makers because its difficult for them to find a job that would be of high wage because of their poor educational attainment or some of them are uneducated. Some people make used their own tricycle for them to provide or buy their basic needs. There is less number of people who work as plumber, caregiver, and security guard. Basically, educational background really matters or has been the factor of seeking good jobs.

INTERPRETATION

Based on the survey, out of 60 families in regards to monthly income, 4 (5.41%) are earning P500-P1,000 a month, 40 (54.05%) of them earn P1,000P4,000, 20 (27.03%) earn P5,000-P6,000, and 10 (13.51%) are earning P8,000P10,000 a month.
ANALYSIS

Most of the people earn P1,000-P4,000 a month. The main cause of small earning is that members of the family dont have stable or permanent jobs depending on the type of work they are in. They are usually self employed which gave them less amount of money from their own. There are some people who earn P5,000-P6,000 who were fortunate to have a stable job on Hotels and some are just summing up the amount of income of the number of families who are employed. Some fortunate families earn P8,000-P10,000 a month. In this matter, he earn this amount because In this matter, they earn this amount because they have stable jobs and at the same time self-employed by rag making for additional income for their family.

INTERPRETATION:

Out of 69 working individuals, 40 (58%) are self-employed, 24 (35%) individuals have casual or contractual jobs, 7 (7%) are un employed (those who are older adults and retired individuals).
ANALYSIS:

Most of the people in the community are self-employed. Some of the people also are having a casual or contractual jobs. These people are basically the one who are Elementary graduates. It is hard for them to look for a permanent or stable jobs because of their poor educational attainment, and that they are not qualified to some job fairs. Some people who are unemployed due to retirement age or they are not capable of working anymore.

GARBAGE DISPOSAL
INTERPRETATION: Based on the data gathered, out of 60 families, 66.15% prefer on burning their garbages, 8.06% use of compost pits, 4.84% are depending on trash/garbage collection, 9.68% throw their garbages on riverside and 11.29% directly throw their garbages on dumpsites. ANALYSIS:

Though the people are already aware of how smoke could damage the ozone layer and health, they still prefer to burn their garbages to avoid mounting and scattering anywhere and believed that burning garbages could really minimize the growth of mosquitoes. Some of the residents dump their garbages on riverside they said to be the nearest dumpsite for them. There are also some residents who just let their garbages collected by garbage collectors. Some families in the community just throw their garbages directly on the dumpsite. We, as a concern student nurses we will explain to them the effects of throwing their garbages in the riverside. This would affect their children and their health status of their family, because this can cause dengue or any disease that can adopt on this case.

INTERPRETATION Out of 60 families that we have surveyed, 29 (49%) verbalized that most of their is waste are disposed in their canal, 6 (10%) dont have drainage system, 20 (34%) have open drainage system, and 4 (7%) have closed drainage system.

ANALYSIS Based on the survey, most of the families in the area do not have drainage system. They will just throw the used water in the canal. They do not prefer to use closed drainage because they cant afford to make their own drainage and maybe they dont have enough area and lack of financial assistance.

ASSESSMENT OF SURROUNDINGS
INTERPRETATION Out of 60 families surveyed, 26 (43%) verbalized that they have a clean environment, 32 (54%) verbalized that their environment is dirty, and 2 (3%) verbalized that their environment is very dirty.

ANALYSIS Based on the data above, most of the families in the community have assessed that their environment is dirty because most of them are not particular in environmental sanitation, although some people are particular but majority of them are not. They need adequate health teaching to know how important environment sanitation is. There are some people also said that their environment is very dirty because the plastic wastes and garbages are very evident.

TYPES OF TOILET
INTERPRETATION Out of 60 families, 14 (23%) have flushed type of toilet facilities, 1 (2%) have Antipolo, 10 (17%) have water sealed, 29 (48%) 0f them usually depend on their neighbors toilet facility and just pay them for 2. There are 6 (10%) who dont have any toilet facility and just defecate on riversides.

ANALYSIS Majority of the families do not have toilet facilities and just depending on their neighbors toilet facility for 2 because most of them lack proper space for them to build and lack of financial assistance .Some of the people also are defecating just on riversides and to those places where there are no residents living.

INTERPRETATION
Based on the data gathered, out of 60 families, 50 (78%) families are using Piped System as their major source of drinking water and there are 15 (22%) families who are using mineral water as their source of drinking.

ANALYSIS Majority of the families are using Pipe System as their major source of drinking water since it is available in their Barangay. They just secure some jars, galloons or bottles that serve as container of the drinking water. And there are some families also who prefer to buy mineral water in containers which cost P25.

INTERPRETATION Based on the survey, 19 (32%) using jar with faucet, 9(15%) using jar without faucet, and 32 (53%) using container with cover.

ANALYSIS Most of the families are using container w/ cover. The families who particularly use jar with faucet are those who have children, because they are concern about their health.

VEGETABLE FOUND IN THE GARDEN


INTERPRETATION Out of 60 families, there are only 22 families who used to have vegetables in their garden. The kind of vegetables that available are Alugbate with 23%, Malunggay and Kangkong with a percentage of 14%, Batong, Butig, Gay and Okra with a percentage of 9%, Ampalaya & Eggplant 4% and Squash with a percentage of 5%.

ANALYSIS Most of the families dont have vegetable garden due to lack of space available and since the land is not legally own by them. Some of the families just put their vegetable plant on pots or any available container like cans. Families who were lucky to have space for vegetable garden usually planted alugbate, malunggay and kangkong. There is less number of families who planted squash, ampalaya and eggplant. Having a vegetable garden could help us save our money instead of buying in the market. Vegetable is safe to eat because it is organic.

LIVESTOCK RAISING
INTERPRETATION: Based on the data gathered, the families of Sitio Kasagingan raised only three kinds of animals. There are only 16 families who used to raise animals, out of 16 there is only 1 (6.25%) family who raised goats and 15 (93.75%) families who raised chicken and ducks.

ANALYSIS: Most of the families dont engage in livestock raising. Their livestock comprises mostly of chickens and ducks can be both fed on poultry layers, pellets and wheat. Goats can be fed by grass or any plants.

INTERPRETATION: Based on the survey, out of 60 families there are only 3 (5%) pregnant women in the community and 57 (95%) are not pregnant.

ANALYSIS: As what we have observed, there is less number of pregnant women in the community, all of them are immunized and are having pre-natal check ups.

TETANUS TOXOID IMMUNIZATION


INTERPRETATION All of the 5(100%) pregnant mothers are immunized with Tetanus Toxoid vaccine.

ANALYSIS The tetanus toxoid (TT) vaccine is given during pregnancy to prevent tetanus to the pregnant women as well as their bodies. Atibodies formed in the body, and after the vaccination, are passed unto the baby and protect him/her for a few months after birth. In the first pregnancy, the doctor will recommend at least two doses of TT vaccine. The first vaccination is given in the first trimester soon after the pregnancy test are confirmed and after the first antenatal appointment . The dose of the TT vaccine is given at least four to eight weeks after the first. The World Health Organization recommends that a third vaccine or dose be given six months after the second one to provide protection for at least five years. Thus pregnant women should have proper prenatal check-up as well as immunization for the protection of both mother and baby. The five pregnant women are immunized with Tetanus Toxoid vaccine for they are aware of the importance and its benefits.

INTERPRETATION Out of 33 families with infants, 29 (88%) are practicing breast feeding while 4 (12%) of them are not practicing. ANALYSIS Based on the survey, most of the families practice breastfeeding. They are aware of the benefits and essentials of breastfeeding through seminars and programs that they have attended in the health centers. It could give a lot of nutrients to the baby due to anti-infective properties of breast milk, breastfed babies have less incidence of or less pronounced symptoms of ear infection, respiratory, illness, allergies, diarrhea, and vomiting. That is why breast milk is really best for babies. But they are some mothers also who do not practice breastfeeding for the reason that they are working mothers.

INTERPRETATION:

Based on the gathered, all of the 5 pregnant mothers in the community have been recommended for the 2nd prenatal visit.

ANALYSIS:
Prenatal care is a type of preventive care with the goal of providing regular check-ups that allow doctors or midwives to treat and prevent potential health problems throughout the course of pregnancy while promoting healthy lifestyles that benefit both mother and child. Prenatal care generally consists of monthly visits to doctor from 28th week of pregnancy, weekly visits to doctor after 36 th week till delivery at week 38-40 and then assessment of prenatal needs and family dynamic. Based on the survey, all the pregnant women in the area have been recommended for the 2 nd visit. The pregnant women should complete their prenatal visits for the appropriate information and pregnancy health status of the mother and the baby.

INTERPRETATION Out of 33 families with infant, 19 (58%) families prepare porridge with salt /sugar, 4 (12%) prepare porridge with any kind of meat, 7 (21%) prepare porridge with vegetables, 2(6%) prepare porridge without mixing anything, and 1 (3%) prepares porridge with egg.

ANALYSIS Preparing porridge with salt/sugar as a weaning food is not that healthy in which it does not contain nutrients appropriate for the body. Porridge with fish, vegetable, meat or egg is considered more appropriate since those provided basic nutrients for the body like protein, Vit. K, Vit. A, Vit. D, and etc. Based on the survey, most of the families are preparing porridge with salt/sugar which is not good and do not give appropriate nutrients for the infants. They said that they could not afford to buy meat to mix with porridge and just put salt/sugar. There are some other families who prepare porridge with vegetable which is affordable. There is less number of families who prepare porridge with meat, fish and egg.

RECOMMENDATION
In each community in a specific area, good health is very necessary. Each family must know the proper garbage disposal, how to clean their area, and if possible they should secure toilet facilities. Family Planning should also be introduce to some other families who are not aware of it. In order to implement these things, health teaching should be conducted to improve the health status of each family in the community. There should always be disciplinary measures because the children need to be guided and so that they could also learn how to decide for their own. Cleanliness in each community is very much important because we concerned individuals is in touch with the goodness and health of the residents. And as concerned citizen, we should be participative in the community activities and services that should be done so that we can be able to help them how to improve their health status and their community in such a way, we will be helping them in cleaning their area and by conducting health teaching. We as the students in this institution Cebu Sacred Heart College, recommended that this community Sitio Kasagingan should improve their environmental status. Provide toilet facilities of their own, practice how to clean their area properly.

CONCLUSION
We, the BSN II Students of Cebu Sacred Heart College conducted a Consolidated Survey at Lawaan III Sitio Kasagingan, Talisay City. During the survey, we noticed that their environment needs environmental sanitation. They are not practicing proper waste disposal. Some of the family are not aware of the Family Planning and most of the children in the community are malnourished. Having this community service whose program mainly implies on Health Teaching such as: Family Planning, Cleanliness of the environment and etc. We the BSN students hope that the things that we have done during the survey will give solution to their problem and could help them to improve their health status. Therefore, we conclude that they as constituents of their community, they should maintain cleanliness in their community in order to prevent any cause of illness to them. They should be aware of the Family Planning methods for them to know how to control the number of family member. And lastly, they should provide healthy/nutritious foods especially fruits and vegetables to their children. We as student in this institution, Cebu Sacred Heart College conclude that this community, Lawaan III Sitio Kasagingan, Talisay City to have a research/survey in the said area for us to know the health status of the constituents of their community.
