Introduction
A comprehensive assessment of learning needs can be extracted from nursing history and physical assessment findings along with information from the clients family. The assessment also includes client characteristics might affect the learning process, such as readiness to learn, motivation for learning, and literacy levels. In addition to extracting data through interview, the nurse should also observe the capability and clients needs. Learning needs can also be identified from client to nurse questions about things they do not know or are not skilled in doing so.

Definition
Notoadmodjo (2003) Behavior is any activity, either directly observed or not be observed by outsiders. Skinner Behavior is a response or reaction to stimulate from outside.

Kinds of Behavior
1. Covert/ passive Behavior Is the response to the stimulus in hidden form (covert) like: attention, perception, knowledge, awareness and attitude. Cannot be observed by others. 2. Overt/ active Behavior Is the response to the stimulus was clearly in the action. It is easily observed.
Health Behavior is the response to the stimulus related to illness or disease, health service system, foods and environment.
