GUIDELINES IN ACCOMPLISHING PROMOTIONAL FORMS


1. Black is the official ink in preparing pupils records / promotional forms. 2. Subject nomenclature (arrangement) must be the same in all forms / records ( Form 1, Grading Sheets, Form 137, Form 138, and SF-5 & 6). 3. Total number of school days 201 (DepEd Order No. 10 s. 2013).

3. Date of Birth 3.1 Should tally with the data in the birth certificate Example : March 11, 2000 = 03 11- 00 4. No. of Days must be the same in all forms. (forms 1, 137, and 138) 4.1. Check the number of days especially for drop-outs and repeaters. If the total number of absences exceeds 20 % of the total no. of days, the pupil is considered dropped from the list. Example : 200 x 20 % = 40 days absent 5. Annual Enrolment 5.1 Annual Enrolment Per section should not change within the school year.

FORM 137 PERMANENT CUMULATIVE RECORD


1. Complete all the required data. Picture of the pupil/student. Elementary school progress Activities and honors Appraisal of pupils behaviour Attendance Record Library Withdrawal and re-entry records Health examinations, height in cm; weight in kilograms Out of school activities Significant note about the pupil for future reference. Home visitation.

CLASS RECORD
Grades should be computed in accordance with the new rating system by subject area. Final grades for every grading period should be rounded off to the nearest whole number. Avoid erasures.

FORM 138
Complete all the data required. Write the name of school and division. The age as of June should be in fraction ex. 8 Avoid erasures in grades. No. of days per month, it should tally with other forms.
Avoid negative comments. General Average must be expressed in two decimal places for nonhonors and three decimal places for honors . - For K to 12 BEC Numerical and Descriptive - For RBEC, no descriptive equivalent, - Descriptive equivalent of Gen. Ave. will only be reflected in SF 5 under Gen. Ave. Column but not in the Card/Form 138 Note : Do not encircle the general average but advisers must affix their initial

DIPLOMAS / CERTIFICATES
Must be alphabetically arranged and same sequence as the list in SF 5 and List of Graduates. Correctness of entries such as name (spelling), dates signatures etc., must be checked. Names must be same as those that appear in birth certificates, SF 5, F-137, F-138, F-1. Copy of List of Graduate: Principal/s assigned by the District/Cluster to check the certificates must indicate OK in each of the names in the list of graduates and his/her signature as checker at the bottom of the document. This shall be placed on top of the pile of diplomas to be affixed with initials of the EPS-checker/s.
