Definition:
Anemia is a condition in which there is
diminished oxygen carrying capacity of the
blood, as a result of reduction in total
circulating Hb/reduction in red cell mass.

The disease anemia is caused by the
absence of any dietary essentials that are
involved in Hb formation or by poor
absorption of these dietary essentials.

Prevalence:
Young children -- 40%
Pregnant women 50%
Non-pregnant women 35%
Adult males 18%

In India 2070%
(it is an important public health problem affecting
people from all walks of life).

Anemia is very widespread, more among
females than males & higher among infants
& children than adults.

Dietary essential causes
By lack of either dietary iron or high quality protein

By lack of pyridoxine(vit-B6) (which catalyses
the synthesis of the haem portion of the Hb molecule)

By a lack of vit-c - (which influences the rate of iron
absorption into the tissues)

By a lack of vit-E (which affects the stability of the
red blood cell membrane)

Deficiency of Cu (aids iron synthesis by influencing
the absorption of Iron, its release from the liver or its
incorporation into Hb molecule)

Most common causes of
iron deficiency anaemia
Diminished food intake
a) Very poor diet
b) Low intake of vit-c

Deficient absorption
1. More intake of Tea/Coffie
2. Due to high phosphate content of cereals
3. Eating clay
4. In malabsorption

Cont.
Loss Hookworm infestation

- Whipworm infestation

- Malarial parasites

- During heavy menstrual cycle

- During childbearing period

- During pregnancy

Clinical findings
Immuno-competence Defects in cell
mediated immunity & phagocytic activity of
neutrophils.

Diminished work performance -

Cognitive development Lower in tests of
academic performance.

Behavioral implications Child is more
disruptive, irritable & restless; Pica, ice
eating & breath holding spells.

Factors affecting absorption
The absorption is being higher with a rice
based diet & lower with millets.
Germination & baking increases absorption;
while it is reduced with tanin, tea & phytate.
Prolonged warming of meals decreases their
vit-c content & hence iron absorption.
The amino acid cystein enhances iron
absorption from vegetables, animal foods &
iron salts.
Coconut milk inhibits iron absorption.
Cont
Iron in meat & liver is better absorbed than
that in eggs & GLV.

Animal protein increases absorption of non-
haeme iron from vegetable sources.

Iron absorption on a cereal diet is decreased
because phosphate & phytates precipitate
ionised iron.

Vit-c taken in divided doses with each meal will
increase iron absorption to a greater extent
than a single large dose with breakfast.
Dietary consideration
Poor appetite should be corrected.

Restricting fried foods/junk foods.

A soft/clear liquid diet.

Supplementation with ascorbic acid.

High protein diet is recommended.

Balanced diet should be given.

Prevention of anemia
Supplimentation

Distribution of iron & folate tablets to
pregnant women during last trimester
(National Nutritional Anemia Prophylaxis Programme)

For pre-school children (as part of MCH services)

10mg iron daily prevent iron deficiency
anemia
Education
Educating both the health functionaries as
well as the general population about
anemia.

Promoting the consumption of iron rich
foods.

Creation of awareness in mothers attending
ANC, Anganawadi centres & creches about
the prevalence of anaemia, its ill effects &
its preventable nature

Cont..
Addition of iron rich foods to the weaning
foods of infants.

Regular consumption of foods rich in vit-c to
promote iron absorption.

Periodical administration of antihelminthic
drugs.

Discouraging the consumption of foods &
beverages like tea/coffie that inhibit iron
absorption especially by the vulnerable
groups like pregnant women & children
Fortification
Fortification of a commonly consumed food
with iron.

Iron fortified salt.

Iron fortification of wheat flour & bread
(in developed countries)

Iron Absorption Enhancers

Meat/fish/poultry

Fruits: Orange, Orange Juice,
strawberries, grapefruit etc

Vegetables: Broccoli, Brussels sprouts,
tomato, tomato juice, potato, green & red
peppers

White wine

Iron Absorption Inhibitors
Red Wine, Coffee & Tea

Chocolates

Soft drinks/beverages

Conclusion
Though anemia cannot be treated by diet,it
can be prevented by taking Balanced diet.

Policies for combating micronutrient
malnutrition must be firmly rooted in food
based rather than drug based approaches.

One should look to farms & not pharmacies
for the solution of nutritional problems.
