DEEPWATER

HORIZON

INTRODUCTION
Deepwater Horizon owned and operated by
offshore-oil-drilling company Transocean
and leased by oil company BP.
Situated in the Macondo oil well prospect in
the Gulf of Mexico.
The rig of deepwater horizon is semisubmersible exploration drilling rig.

ACCIDENT
CHRONOLOGY
9 April : The final section of the well
is drilled
10 April: The cement job started to
seal the
wellbore from the
reservoir sand.
20 April: Integrity Test was carried
out
Positive
pressure
test
(successful)
- Negative pressure test ( result

17.30 : First negative pressure test on

drill line
was tested. It FAIL.
18.30 : Second negative pressure test on
kill
line was tested. They assume
succesful
but pressure on drill line
still high
which due to bladder
effect.
20.00 : Crew start the temporary
abandonment of the well process.
21.30 : Higher differential pressure was
noticed.
21.40 : Mud overflows onto the rig and
blowout started.

CAUSES OF INCIDENTS
Well integrity was not established or failed
-Annulus cement barrier did not isolate HC.
-Shoe track barrier did not isolate HC.
HC entered the well undetected and well control
was lost
-Negative pressures test was accepted
although well integrity had not been
establish.
-Influx was not recognized until HC were in
riser.
-Well control response actions failed to regain
control of well.

HC ignited on the deepwater Horizon

-Diversion to mud gas separator resulted
in gas venting onto rig.
-Fire and gas system did not prevent HC
ignition.
Blowout preventer does not seal the well
-BOP emergency mode does not seal well.

AFTERMATH
o 11 people lost their lives and 17 people injuries.
o Releasing about 4.9 million barrels (210 million US gal:
780,000 m3) of crude oil making it the largest marine oil
spill.
o The oil spill contaminated more than1,100 miles of
coastline , at least 1,200 square miles of the deep ocean
floor and 68,000 square miles of surface water.
o More than $11.6 billion has been paid to Gulf-area
individuals and businesses who have suffered economic
damages or medical issues as a result of oil spill.
o The Gulf of Mexico commercial fishing industry was
estimated to have lost $247 million as a result of post-spill
fisheries closures.
o It cost about BP $4 billion to clean up the mess, and
another $4.5 billion in penalties.

COMPARISON
BETWEEN TRUE EVENT
AND THE MOVIES

IN THE MOVIE
Based on the movie Deepwater Horizon, it
appears that BP is the one to blame.
Vidrine and Kaluza that represent BP company
does not want to conduct a cement log process
due to shortage budget.
On the first negative pressure test, it shows that
the pressure at drillpipe is high (1400 psi) but there
is no any leakage oil and gas. Vidrine assumes that
the increasing of pressure due to bladder effect.
Jason seems agree with Mr. Jimmy to not perform
completion of drilling.

TRUE EVENT
The company that should be blame are BP ,
HALIBURTON and TRANSOCEANS. Each of the
company contributed human error that causes the
accident to occur.
BP try to reduce the cost of completion well by not
conducting cement log and use only 6 stabilizers
out of 21.
HALIBURTON provides poor condition of cement to
perform the isolation of the hydrocarbon in the
wellbore.
Facilities and utilities provided by Transocean are in
bad shape and most of it need to be repair.

SUMMARY
The accident report concludes that no single
action caused the incident.
It was a culmination of a complex interaction
of mechanical failures, human judgements,
engineering
design,
operational
implementation and team communication.
