Periarticular Fractures Around the Hip and
Knee: Fix or Replace?

FIGURE 1 . Acetabular fracture with medial

displacement involving quadrilateral plate
after low-energy trauma in a woman 86 years
of age illustrated by conventional radiographs
(A) and computed tomography slice (B). The
fracture was reduced and fixed with two
plates and screws; postoperative radiograph
(C). One week later, secondary displacement
with medialization of the femoral head
resulting from insufficient fixation combined
with weak bone (D).

FIGURE 2 . Supra-/intracondylar femoral

fracture in a woman aged 79 years with
severe osteoporosis (A). The fracture healed
after fixation with a combination of an
intramedullary nail and separate screws. Eight
years later, the patient sustained a proximal
tibial fracture after low-energy trauma (B);
otherwise, the patient is healthy, mobile, and
living independently. There was no
antiosteoporotic treatment in between the
fracture events. Fracture treated with a
combination of a conventional angle-stable
plate and injectable calcium phosphate
around the screws for augmentation (C).
