ASSESSMENT OF BREASTS

BREAST
Thebreastistheupperventralregionofthetorsoof
aprimate,inleftandrightsides,whichin
afemalecontainsthemammaryglandthat
secretesmilkusedtofeedinfants.
Bothmenandwomendevelopbreastsfromthe
sameembryologicaltissues.However,atpuberty,
femalesexhormones,mainlyestrogen,promotebreast
development,whichdoesnotoccurinmen,duetothe
higheramountoftestosterone.Asaresult,women's
breastsbecomefarmoreprominentthanthoseofmen.
BreastAssessment-Inspection
Seatclientonsideofbedorexamtable
Inspect for:
Visiblemasses(changeincontour)
Skindimpling
Nippleretraction
Redness
Instructtheclienttoraiseherarmswhileyoucontinuetowatchthe
breasts.
Anunderlyingmalignancycanfixtheskininplace.
Raisingthearmswillhighlightthesechanges.
Haveherflexthepectoralismajormuscles.Asimpleway
todothisishaveherplaceherhandsonherhipsand
squeezeinward.Anotherwayishaveherplaceher
palmstogether(prayingposition)andsqueezethepalms
together.
Ask her to raise her arm over her head.
Thishastheeffectofstretchingand
tighteningthepectoralismajormusclethat
liesdirectlyundeneaththebreast.
Functionally,thisplacesthebreastona
fairlysolid,fairlyflatsurface,makingit
easiertopalpateabnormalities.
PALPATION
Withthepatient'sarmraisedoverherhead,palpatefor
lumps,massesorthickenings.
Breasttissueisnormallysomewhatnodularor"lumpy,"
particularlyintheupperouterquadrant.
Lookforadominantmass.
Somehavesuggestedthatyouarelookingfor"amarble
inabagofrice."
Palpatethebreastusingtheproximalandmiddle
phalangesofthefingers.
Thepalmofthehandistooinsensitivetodetectsubtle
changesinbreasttexture.
Thefingertipsaretoosensitiveandwillfocusonthe
normalgranularityofthebreasttissueratherthanthe
moreworrisomemasses
Whenbreastsarelargerorpendulous,itmaybeusefulto
usetwohands,compressingthebreasttissuebetween
them.
Check the axilla for masses or palpable lymph
Itisrelativelycommontofindpalpablelymphnodesin
theaxillaofnormalpatients.
Palpableaxillarylymphnodesthatarepresentbecause
ofanunderlyingmalignancywillnotgraduallyregress
overtime
Check the supraclavicular area for palpable masses.
Thesupraclavicularareacanbeanareaofspreadof
breastmalignancy.
Palpablemassesorlymphnodesinthisareacanbea
signofunderlyingmalignancy
Stripping the ducts towardthenipplewillcauseanysecretionsto
beexpressed.
Thisshouldbedonefirmly,butnotsohardastocausediscomfort
orpinching.
Witheffort,youwillalmostalwaysbeabletobringadroportwoof
breastsecretionstothesurface.Thisisnormalandthesecretions
willbeclear,milky,orhaveaslightgreenishtinge.
Bloodydischargeisalwaysconsideredadangersign.
Largeamounts(manydrops)ofsecretionsarenotconsidered
normalandusuallyrequirefurtherinvestigation.

other breast and repeat the examination.
Gobacktothefirstbreasttocomparefindingsfromonesidetothe
other.
Athickeningthatissymmetricallypresentinbothbreastsisusually
ofnosignificance.
Athickeningthatispresentinonlyonebreastismoreworrisome.
The breast examination can also be performed with the patient
reclining.
Thepatientmaybeflat,orsemi-recliningwiththeheadandtrunk
partiallyraised.
