CASE PRESENTATION

C
Who?
There are many residents within the community that have
disabilities, whether they are independent or dependent.
Due to it being a rehabilitation and nursing home facility, there
are a variety of short-term and long-term residents.
There are those with short-term and long-long term goals, but
many are not able to attain them.
One resident in particular, who was almost rejected from the
facility because of her weight, opened up an opportunity for
the facility.
This resident came into Genesis Health Care weighing at 455
pounds.
Background Information
The client was born on July 24th, 1952.
Her mother and father are both Lumbee.
The client was born and raised in Lumberton.
She grew up with three older brothers, an older sister, and a
younger brother.
She went to Piney Grove High school, dropped out after the 9 th
grade, and she loved to dance.
When she got into her early 20s, she got a job at an
embroidery factory trimming clothing.
She got married shortly after entering her 20s.

Together, her and her husband had 2 boys.
She was 56 years old when being admitted into the facility in
July of 2008.
She was admitted as morbidly obese with chest and back
problems.
She began to eat intensively once her husband passed two
years prior to entering into the facility.
Being morbidly obese and not being mobile, putting her on a
food diet was the best option for her in hoping she is able to
lose enough wait and begin to learn to walk again.
Services Provided
The facilities goal was to set this resident up on a safe gradual weight
loss plan by controlling her portioning size and the amount of times she
eats a day.
The resident, director of nursing, dietitian, social worker, and cook are all
working as a team to ensure the residents weight loss goal is effective.
The team specialists provide education and meal portioning control.
She is only allowed one meal tray per meal.
In 2 years, she went from 455 lbs to 395 lbs.
Her family members sometimes impede her weight loss goal by
supporting her previous bad eating habits.
Strengths and Limitations
The resident has many strengths. Some examples are that she is alert and oriented,
is able to feed herself and eat solid foods.
Her ultimate limitation would be her weight.
Her weight causes her to have type 2 diabetes, chest and back problems, and not
being able to walk.
Interventions and Goals
The only interventions the facility can provide are the handling
of her food and educating the resident, friends and family
members on the severity of her condition.
Informing them on the positives of losing weight, and the risks
of gaining more weight.
The ultimate goal would be for her to lose enough weight for
her to gradually learn to walk again.
Observations
I believe the family systems framework has played a part
within goal setting and interventions.
If the resident became morbidly obese due to her husband
passing away, and if the family continues enabling her with
bringing her unhealthy foods, then they are aiding the problem
in a way.
Some biases I believe I had going into this was that she would
lie about her eating and what her family brings her because of
her addiction to food.
Our values differed by the importance of being independent.
Observations Continued
Her husband passing was definitely a traumatic hardship she
went through, and could possibly still going through it.
The staff have monitored her weight loss and have seen that she
has gained some weight in the past 8 months.
I think the agency is doing everything they can to monitor the
resident and ensure that they are doing their part in educating
the resident and their family members.
