 Incident reporting
Reporting

The Site/construction manager or authorized supervisor

is accountable for safety, incident reporting and
investigation. (chapter 4.3)

All employees
can file a report in the Incident database
no special access rights are required.

Special access rights are required to alter reports and to view

personal info etc.

However, each BU have decided upon on how to report incidents,

due to this: the reporting structure will differ from BU to BU,
(call center, technicians, safety representatives, HSE department,
safety professionals, manager
