result in an injury
HEINRICHS DOMINO THEORY 6

If there is an injury ALL 5 factors are involved.

If one of the factors in the sequence leading to an
incident is removed,
loss can be prevented: e.g Eliminate UNSAFE ACTS /
UNSAFE CONDITIONS makes the action of preceding
factors ineffective

47

Are foremen critical for safety?

Its true that no other person on the site has

more responsibilities for safety than the site
foreman.
Similarly no person has more direct influence
over the workers than the site foreman.
Therefore the foreman is the only one who
can, if he chooses to do so, ensure that the
(2 pagine)

workers do their jobs in a safest and most

efficient manner possible.
Mod. PPT1 - Rev. A
Aims for this training 48

Objectives 49

Through the information you will gain during

this training session and through open and
honest discussions we will:
Achieve a common understanding
and

Safety Basics 51

What is the Real Reason

If workers are allowed to break safety
rules?
If workers arent required to follow safe
work practices and procedures?
If workers are allowed to work unsafely?
If workers do not have the proper tools
(2 pagine)

and equipment to do their jobs safely?

Mod. PPT1 - Rev. A

WHY?
