1.

Teaching-learning Process (40%)


a. Prepares lesson plans and Daily logs of activities including
appropriate , adequate and updated instructional materials.
b. Facilitates the learning of learners through functional daily
lesson logs and innovative teaching strategies and cooperative
learning is evident.
c. Maintains discipline of learners inside the classroom guided by
the rules and regulations.
d. Monitors daily attendance and maintained a safe and orderly
environment with respect to all learners
2. Student's Outcome(30%)
a. Keeps accurate records of grade/performance in every
grading period: (Class Record, Portfolio, School Forms,
Form 137, Form 138)
b. Monitors, evaluates and maintains Class/Student's
progress: (Portfolio, Journal, Test Results, Outputs)
c. Accounts learning outcomes vis--vis goals and targets:
(NAT/Achievement Test, Drop-Out, Promotion, Pre- Post
Test)
3. Professional Growth and Development (10%)
a. Attends seminars, workshops and trainings for teacher during
the rating period
b. Conducts problem/ classroom based action research
c. Participates actively in any professional or civic organization4.
d. Renders the following technical assistance:
*provided assistance to co-teachers in improving their teaching competence
*assisted school administrators in planning and managing in-service training
4. School, Home and Community
Involvement(10%)
a. Conducts quarterly PTA meetings/ conferences/
consultations
b. Conducts home visitation of 3 % of the students in his class
c. Participates in at least 1 outreach programs (such as gift-
giving, feeding program, tree planting, fun run etc.)
