    Avian influenza, also known as bird flu, is caused by influenza viruses that normally infect birds. While human-to-human transmission of the H5N1 strain has been reported, it is generally contracted through direct contact with infected live or dead birds. The clinical symptoms can include fever, cough, sore throat, and shortness of breath. Complications may include severe pneumonia, acute respiratory distress syndrome, and multi-organ failure. Treatment focuses on supportive care, antiviral medications, and antibiotics; however, the mortality rate of H5N1 avian influenza in humans is over 50%.

    Human to human transmission of influenza
    A (H5N1) has been suggested in several
    household clusters and child to mother
    transmission
    Clinical Manifestation
    The clinical spectrum of influenza A (H5N1) in
    human base on description of hospitalized
    patients
    Incubation period may be longer than human
    influenza 2-4 days, upper limit 8-17 days
    History of contact with poultry:
    Visited a poultry farm
    A household raising poultry; or a bird market
    Contact with a known or suspect human case of
    imfluenza A (H5N1) in an H5N1-infected area within
    10 days of symptom onset
    Temperature of >38 C
    Influenze like illness with lower respiratory
    symptoms
    Dry Cough
    Sore throat
    Nasal congestion/ rhinorrhea
    Shortness of breath
    Sneezing
    Headache
    Body aches/myalgia
    Fatigue
    Irritated eyes
    Acute Respiratory Distress syndrome (ARDS)
    Tachypnea
    Diarrhea without blood
    Abdominal pain
    Radiographic changes:
    Diffuse
    Multifocal, or patchy infiltares
    Interstitial inflitrate
    Segmental or lobular consolidation
    Pleural effusions are uncommon
    Pneumonia

    Management
    Most hospitalized patients requires ventilatory
    support
    Broad spectrum antibiotics
    Antiviral agents corticosteroids
    Adamtane group:
    Amantadine
    Rimantadine
    Neuromidase inhibitors
    Oseltamivir, 2 x 75 mg/daily for 5 days within 36-48 hours
    after the onset of illness
    Inhaled zanamivir
    Complication
    Multiorgan failure
    Renal dysfunction
    Cardiac dilatation
    Supraventricular tachyarrhythmias
    Complication have induced ventilator:
    Pneumonia
    Pulmonary hemorrhage
    Pnemothorax
    Pancytopenia
    Reyes syndrome
    Sepsis
    Sputum sometimes bloody
