DT is a 49yr old man who started statin therapy last

year
PMH : Hypertension and Dyslipidemia
Social history: 1-2 beers per day and smokes. No
exercise
Drug history : Hydrochlorothiazide (HCTZ) 25mg daily
, Simvastatin 20mg daily .
Vitals : BP 136/84 mmhg , weight 75kg, height 70”
,waist 30”
Fasting lab reports : TC 187 mg/dL , HDL –C 40mg/dL
, LDL-C 120 mg /dL , glucose 105mg/dL , hs-CRP 4.1
mg/L
All other labs are within normal limits
1. Assess cardiovascular risk and lipid values
• The risk of having a heart attack or stroke is 12.2 %.
• The score of a healthy person with the same age, sex and ethnicity is 4%
• All the lipid values are normal .

2. Identify appropriate goals of therapy


• To control his blood pressure (136/84mmhg) near
normotension ( 120/mmhg ) and dyslipidemia by
life style modifications and drug therapy

• Management of complications which may arise

and consult with doctor regularly
