Objectives
1. To provide essential knowledge of the services related to the
understanding of structures and functions of one’s
environment.
2. Make the students understand the relationship of the
physical, socio-cultural and spiritual environment to his/her
total preparation as loving and caring health professional.
3. Acquire sufficient skills, knowledge, attitude and able to
demonstrate beginning professional competencies based on
the eleven key areas of responsibility of the nursing
curriculum namely: safe and quality nursing care,
management of resources and environment, health
education, legal responsibility, ethico-moral responsibility,
personal and professional development, quality improvement,
research, record management, communication and
collaboration and teamwork.
4. Develop among students critical thinking in order for them
to make wise decision when faced with problems.
5. Assists student’s affectivity utilizes the nursing process as the
basic foundation for nursing practice.
6. Prepare the student for leadership role in any setting,
community health care institution in nursing education.
7. Understand his/her role as member of a society.
8. Contribute to the attainment of health for all and
consequently for nation development.

SPECIFIC OBJECTIVES

• To develop appropriate methods in obtaining data about the


community about the health status of a member of the family as
individuals and the community as whole.
• To formulate an accurate assessment for the community in
identifying health problems and have a significant, attainable and
community-based solution.
• To participate in developing health promotion and plans for the
community thru active participation of the community to
improve the health status of the whole.
• To coordinate with other agency such as the LGU’s, MHO, for
better implementation of the programs of the Department of
Health.
• To be the health practitioner and provide quality promotion of
health in developing activities regarding health problems.
• To educate and increase their awareness on how to utilize
available resources within the community.
• To evaluate strengths, weakness, opportunities, and threat related
to risk and potential level of families within the barangay.

INTRODUCTION

“The Intensive Nursing Practicum is designed for the senior


nursing students of an accredited baccalaureate program (BSN) as a
culmination of their knowledge, skills and experience. Since 1993 the
school of nursing has offered a required course in which students
participate in a reflective experience that provides the context to
integrate class room and experiential learning in an involving
professional identity. Over the years the number of students have opted
to complete course requirements through work in an international
setting. INP is the live application of your nursing knowledge and skills
in real life situations and events. The students will be exposed in the
community for a month to practice professional nursing competently
in diverse settings, utilizing caring, critical thinking and therapeutic
nursing interventions with individuals, families and
communities.”(Husson University)
Gathering all data that was from the community through
direct approach with them, we will be able to advocate health by
imparting health teachings and apply programs of the government to
the community especially to the families who is in need. By then, we
can determine the condition of the community and implement specific
remedy to a specific problem.
On the succeeding pages, there you’ll find information regarding not
only the names, ages and other statistical data of Brgy. Poblacion West,
General Tinio, Nueva Ecija . But this will also carry out as an opening
for us; a future health care providers in imparting knowledge for the
health wellness of individuals, family, community, society and as whole.

HISTORY OF BARANGAY

Barangay Poblacion West was created way back


in 1981. It was known as Barangay 7 Poblacion West,
General Tinio Papaya and Santiago Bolisay served as the
first Barangay Captain of the said barangay. Then he was
succeeded by kind and generous chairmans such as Isauro
Mangahas, Antonio Bolisay, Crispin Pajarillaga and Ruben
Garcia.
The Barangay was just a simple community way
back then with vegetable planting as their way of living.
Until Primitivo Bolisay became the Barangay Chairman
and divided it into 4 purok or zones. Succession continues
and the barangay becamed more developed through Dan
Pajarillaga, Benito Bolisay and the first chairwoman of the
barangay, Arlene Fajardo.The present barangay chairman,
Joel Bote (2010-present) made the barangay more lively.
The barangay's name changed and became the Barangay
Poblacion West. He also made another zone in the
barangay to make the barangay a 5-zoned community.

Our Intensive Nursing Practicum (INP) Affiliation, started on


the 4th day of January 2018, we the level IV students together with our
Clinical Instructors made a courtesy call to the Municipal Mayor Hon.
Engr. Ferdinand P. Bote, Municipal Health officers Dr. Marr Enrico
R. Ador and staff of RHU of General Tinio ( Papaya ) N.E for a
courtesy call. We proceed to our assigned Barangay, Poblacion West
were we also made a courtesy call to the Brgy Captain Ho. Joel B. Bote.
Orientation and ocular visit to the area followed next. The task were
distributed equally to each Head Nursing students ,each one has their
own task to comply. We need to survey every household of the whole
barangay ,the barangay is composed of 5 puroks, 2 of which has the
highest number of household and population so it was assigned to 4
Head Nursing Students.
Datas that we need to gather in our survey/ homevists were
general information of the Barangay in terms of their population,
family data and we gave special attention to the categories that is a
contributory factors of different programs of the Department of Health
like OPT (Operation Timbang) children 72 months and below, EPI
(Expanded Program on Immunization) children 0 to 11 months.

Family Planning Acceptors (Married Women of Reproductive


Age), mortality and morbidity cases and other environmental
factors that affect the health of the community.
The NEUST College of Nursing includes Blood Type
profiling in the activity of INP/CHN, on January 8,2018, we
proceeded to our activity each family head and their spouse in
order for them to know their blood type and for future reference
of the barangay for blood donation purposes .
On January 19, 2018, we made a garden for herbal plants
at the side of the Barangay Hall and Barangay health Center.
Twelve (12) herbal plants of approved by the Department of
Health where planted these are Lagundi, Herba Buena, Ulasimang
Bato, Sambong, Ampalaya, Bawang, Akapulko, Tsaang Gubat,
Niyug-niyogan, Bayabas, malungay and Tawa-tawa were planted.
During our INP Affiliation, we were able to conduct 3
feeding programs. Feeding programs were carried out based on the
result of the Operation Timbang (OPT). Having this list, we were
able to identified the who were normal and malnourished children
with the help of our clinical instructors. We were able to know
who will be given extra attention during the feeding program
which was done every Tuesday of our stay in the community.

NARRATIVE REPORT

We discussed important matters during the program like


Herbal Plants and its use, family planning, expanded program of
immunization, tetanus toxoid and proper child rearing and
nutrition. We also had a raffle draw for the entertainment of the
audience.
The students conducted Mother’s Class last January 22,
2018 together with 3rd feeding program attended by the married
women of reproductive age and of course with the presence of our
Rural Health Midwife Ms. Tessie S. Mangulabnan, Barangay
Captain Joel B. Bote with the barangay officials and barangay
health workers.
On January 25 2018, we had our Community Forum that
was held at the Senior Citizen/Wellness Center beside the Rural
Health Unit of General Tinio, Papaya where we presented the
actual datas that the Level IV Nursing students gathered with the
supervision of the faculty members of the College of Nursing
within the barangay of Poblacion West in front of the Mayor,
Rural Health Unit team, Barangay officials and the Barangay
Health Worker. The program went smooth, the officials of
General Tinio given us a pleasing feedbacks where they stated their
appreciation so for us it was a job well done.
The Level IV students need to conduct a case study where
we need to choose individual cases coming from the barangay and
present it to school as one of our requirements. We are given time
to interact with the opted patients to gather all the datas we need
to accomplish it.

NARRATIVE REPORT

On January 30, 2018 we conducted a Gift Giving


together with the 4th Feeding Program for the children of
Barangay Poblacion West ages 0 to 10 years old. We conducted a
program where we taught them a proper hygiene and gave them
donated slippers on different sizes, coloring books with colors,
hygiene and foods.
Intensive Nursing Practicum is somewhat unforgettable
to each of us. We gained knowledge and built a lot of memories
that will never last into our mind. It test our batch if how far we
can go just for us to accomplished the assigned task in every
exposure at the area. But just like a story, it will always has an
end but our batch will cannot deny the fact that it leave marks in
our heart that when it flashbacks in our mind it will create
mixed emotions in our faces.

Purok 1 is represented by the color violet houses. There are 53 total

houses. In Purok 1, there 2 landmarks the gethsemane church and
Gethsemane Christian College School. There’s also a mini parlor shop

Purok 3 is represented by the color orange houses located at the right
quadrant of the map and has a farm called tomana which is planted
with pechay. Our purok is composed of 107 houses. 65 concretes, 19
semi concretes, 9 makeshift, 6 are made up of woods. There is 1
abandoned house, 5 rest house, 2 underconstruction and a total
surveyed of 99 houses.

houses consisting of 70 surveyed houses, 7 unserveyed houses, 3

Based on the gathered data on the family structure, the


highest percentage with 363 (71.15%) are nuclear family and
147(28.85%) are extended family.
It implies that the families in Brgy. Poblacion West had a
close family ties where in the immediate member or relative resides with
them to aid or help one another. The proximity closeness of the family
provide security and protection for each member. Nuclear family has a
better chance of satisfying its basic needs. This is due to the resources
that are limited to its immediate family members only. On the extended
family, there is more socialization time with the relatives which binds
the companionship closer.

Based on the gathered data on the family size, the highest


percentage with 48.54% is the small family, second is the medium family
with a percentage of 46.59% and the least is the large family with a
percentage of 4.87%. Data shows that most of the families in Poblacion
West is a small family which mostly composed of a mother, a father and a
child.
It implies that those having smaller families, the child receives
more attention of higher quality from parents that will result to higher
achievement of child because of enough proper care, guidance, supervision
and can send their children to proper school.

Aging family are ages 61years old and above. With


teenager are the families whom the parents has a daughter/son ages
13 to 19 years old. Middle Age family, are the parents ages of 45 to
60 years old. With pre schooler are the families who has a 4 to 5
years old child while the schooler family are the one who has a 6 to
12 years old children. Launching family is being determined when
the eldest son/daughter of the couple are already married or has an
own family. Beginning Families, are the newly wed couples whom
starting a family.
Based on the gathered data on the stages of family
development, highest percentage with a total number of 108
(21.05%) are the aging family. Followed by the with teenager family
having a total number of 96 (18.71%). Next is the middle family with
a total number of 92 (17.93%) and the least is the beginning family
with a total number of 17 (3.31%).
It implies that the officials must give more attention to
the elder persons by giving more programs pertaining to the
attainment and maintenance of good health.

206 (11.06%) belongs to 15 to 20 yars old. Followed by the 1

Education is the key to success in life, when you are


a educated person the higher chances of opportunities that you
might get in life.
Based on the gathered data on the educational
attainment, out of 1862 individuals the highest percentage with
total of 333 (17.88%) are the High school Graduate followed by
the College Graduate 309 (16.60%) and the Elementary Level
has a total number of 248 (13.3%). The least percentage is the

reproductive age, the highest with a total number of 31 (10.76%) are the ages
27 to 28 years old. Second is 22 (7.64%) are ages 33 to 34 years old followed by
20 (5.37%) are the ages 43 to 44 years old and the least are ages 49 years old
with 5 (1.03%)
It implies that proper education with regards to importance of
reproductive health must be reinforce to increase the awareness of having a
good health when it comes to this.

(63.19%) are non acceptor, 95 (33.33%) are acceptors and 10 (3.
47%)are pregnant.
Family planning acceptors wherein they are using various
type of family planning methods like pills, injectable, bilateral tubal
ligation, Lam and others.
It only implies that there will be a high possibility of an
increased in the population in the following years due to the high
percentage of married woman of reproductive age that are non
acceptors of family planning .

Based on the gathered data on the tetanus toxoid, on


going has the highest percentage with 8 (80%) followed by the no
immunization and fully immunized pregnant women with 1
(10%).
As a health care provider, it is good to know that
majority of the pregnant women are engaged with the said
immunization because it implies that they have the knowledge
and capabilities to protect themselves as well as their baby in the
womb.

Based on the gathered data on the family planning


methods, family planning acceptors pills gathered the highest
percentage with a total number of 45 (47.37%) followed by Bilateral
Tubal Ligation with 24 (25.26%) and calendar with 7 (7.37%). The
least in the category is use of condom and withdrawal as a method
of contraception both gathered 2 (2.11%).
It implies that the most common use is the artificial
type of contraception such as pills, which provides higher accuracy
and reliability.

houses which may indicate a proper lighting in the

Barangay that makes their travel easier.
7. Active participation of the people in the community
for all the program conducted in the barangay.

OPPORTUNITY

1. Accesible Health Center for availing health programs are


well implemented. RHU are closely monitoring the
community people with regards to every health care needs
of every Purok.
2. Municipal Health Center had good facilities and lots of
resources when it comes to stock of medication that might
be able to used in times of necessities.
3. Educational Attainment in the total no. of population
have an education and degree in colleges which may
suggests a higher chance of getting a good employment.

1. Most of the household numbers had an open


drainage system which may result to breeding sites
of insects.
2. Most of the family owned pets that may not be
vaccinated with anti-rabbies which may results to
the spread of virus from vector to vector and to
human.
3. Some of the house used burning as their method of
garbage disposal which may cause possible health
related problems.
