• Intubasi karena serangan karena asma dalam setahun
asma terakhir
• Pneumotoraks dan/atau • Tidak teratur berobat sesuai
pneumomediastinum rencana terapi
• Serangan asma berlangsung • Berkurangnya persepsi tentang
dalam waktu yang lama sesak napas
• Penggunaan steroid sistemik • Penyakit psikiatrik atau
(saat ini atau baru berhenti) masalah psikososial.
• Alergi makanan
Steroid sistemik (oral atau parenteral) perlu diberikan pada
awal tatalaksana meskipun pada penilaian awal
serangannya ringan sedang

Metered Dose Inhaler (MDI)
Canister, an aluminium can with metering valve and valve stem, it contains micronized
drugs suspension, with multiple doses + propellant (HFA or else)
Advantages
• Convenient
• Less expensive
• Portable
• No drug preparation
• No contamination
Disadvantages
• Coordination essential
• Patient activation required
• Large pharyngeal deposition
• Difficult to deliver high doses
• Not all medication available

INHALER
BABYHALER
Dry powder inhaler (DPI)
• the power source is the flow of inspiration /
inhalation of the patient
• breath-actuated inhaler, no propelan
• effort dependent
• less oropharynx deposition
• not suitable for under 5 children
• for older children easier to use than MDI
• no need of spacer, easy to carry

PENGGUNAAN
DPI use - Diskhaler

•Push the small lever maximally, until ‘click’
Inhalation step
• hold it up-right, exhale slowly
• put the mouthpiece between lips tightly,
inhale as fast & as strong as possible
