▹ Elevación del punto J en 2
derivaciones contiguas ≥ 1 mm
en todos las derivadas.
▹ Excepto en V 2 a V 3
▸ ≥2 mm en hombres ≥40 años
▸ ≥2.5 mm en hombres <40
años
▸ ≥1.5 mm en mujeres sin
importar la edad.

• IM INCIDENTE: Primer IM del individuo
•RECURRENTE: IM que se produce después de 28 días de IM incidente.
• REINFARTO: IM que ocurre dentro de 28 días de un IM incidente o
recurrente.
• MINOCA: IM con arterias coronarias no obstructivas / <50% estenosis
(6-8%), resultado de un mecanismo isquemico responsable. Se debe
descartar Enfermedad aortica aguda.

1.Troponina por encima de la limite superior

o Síntomas de IMA
o Nuevos cambios isquémicos en el ECG
o Ondas Q patológicas
o Imágenes de nueva pérdida de miocardio viable o una
nueva anomalía del movimiento de la pared regional.
o Identificación de un trombo coronario mediante
angiografía, incluida la imagen intracoronaria o
mediante autopsia.

MI TIPO 2
1.Troponina por encima de la limite superior y evidencia de un
desequilibrio entre OFERTA/DEMANDA O2 no relacionada con la
aterotrombosis coronaria aguda
2. Con al menos 1 de los siguientes:
o Síntomas de IMA
o Nuevos cambios isquémicos en el ECG
o Ondas Q patológicas
o Imágenes de nueva pérdida de miocardio viable o una nueva
anomalía del movimiento de la pared regional.
o Identificación de un trombo coronario mediante angiografía,
incluida la imagen intracoronaria o mediante autopsia.

MI TIPO 3

oPacientes que sufren muerte súbita, con síntomas sugestivos de

isquemia miocárdica acompañados de supuestos nuevos cambios
de ECG isquémicos o fibrilación ventricular y NO pueden obtenerse
muestras de sangre para biomarcadores.

o O diagnosticados en examen de autopsia.

MI TIPO 4

LESIÓN MIOCARDICA POR PROCEDIMIENTO

o Aumento de los valores de ctn en pacientes con valores de

referencia normales.
o Aumento de los valores de ctn> 20% del valor de referencia cuando
el valor previo esta por encima de limite superior pero está estable o
en descenso.

MI TIPO 4 A RELACIONADO CON PCI ≤48 HORAS
1. Elevación de los valores de cTn> 5 veces el limite superior.
2. En pacientes con niveles elevados de cTn (estables o
descenso) debe aumentar > 20% el basal 3y ser al menos 5
veces el limite superior.
3. Mas uno de los siguientes:
oNuevos cambios isquémicos en el ECG.
oDesarrollo de nuevas ondas Q patológicas.
oImágenes de evidencia de una nueva pérdida de miocardio viable o
una nueva ano-malía del movimiento de la pared regional.
oHallazgos angiográficos compatibles con una complicación
limitante del flujo del procedimiento.

MI TIPO 4 B TROMBOSIS DEL STENT ASOCIADA A PCI

1. Se utilizan mismos criterios de IM tipo 1

2. Dependiendo del momento de aparición puede ser

• Aguda < 24 hs
• Subaguda 24 hs – 30 días
• Tardía 30 días – 1 año
• Muy tardío > de 1 año

MI TIPO 4 C REESTENOSIS ASOCIADA A PCI

1. Reestenosis focal o difusa.

2. Lesión compleja asociado a criterios de IM tipo 1

MI Tipo 5 relacionado con CRM ≤48 horas
1. Troponina > 10 veces del limite superior en pacientes con valores de
ctn basales normales.
2. En pacientes con niveles de T elevados pero estables o en descenso,
debe > 20% y ser > 10 veces el LS.
3. Mas uno de los siguientes Criterios
oDesarrollo de nuevas ondas Q patológicas
oLa angiografía documentó una nueva oclusión del injerto o una
nueva oclusión de la arteria coronaria nativa
oEvidencia por imágenes de nueva pérdida de miocardio viable o
nueva anomalía del movimiento de la pared regional.
