Onset of pain

Fluid w/in the pericardium puts


pressure on the heart

The pressure prevents the heart


from expanding completely &
filling to capacity w/blood

Less blood leaves the heart

Too little O2 reaches the tissue

Clinical manifestation:
-Faster heartbeat
-Rapid fall in BP
-Faster breathing
-Swollen neck veins
-Loss of consciousness
-Sudden death

Diagnostic tests: S/Sx: Complications:


-Chest pain(Sharp,
-ECG piercing chest pain -pericardial effusion
-EKG
-Cardiac MRI .

left side of the chest,
-cardiac

-Physical exam which is generally -multisystem failure
-Laboratory tests: more intense when
CBC breathing in)
BUN -pericardial friction
Serum Electrolytes rub
Creatinine Levels -dyspnea
Erythrocyte -low grade
Sedimentation fever,sweating,chills
Rate(esr) -Joint pain
C-reactive -Arrhythmias
Protein(CRP)levels
Nursing diagnosis:

☻Acute pain r/t tissue inflammation and presence of


effusion
☻Activity intolerance r/t imbalance between o2 supply and
demand
☻Risk for decreased cardiac output
Medical management: Nursing management:

-(NSAIDs) to decrease the -Recognize the s/sx of


increasing pericardial
pain and
effusion and cardiac
inflammation(such as
tamponade
ketorolac,ibuprofen) -Monitor intake and output
-Corticosteroids Maintain adequate fluid
(prednisone) intake
-antibacterial -Monitor-blood pressure,
(vancomycin) Pulse temperature,
-Surgical respiration, chills diaphoresis,
mgt(pericardiectomy/peri jugular pressure.
-Assist the patient with the
cardiocentesis)
use of spirometer and with
deep breathing exercise
-Maintain the pt. in a high
Fowler’s position to promote
lung expansion,and
administer pain drugs as
ordered
