MOTIVO DE
CONSULTA
‘‘No tenía fuerza en las
piernas’’
 ENFERMEDAD
4 ACTUAL
Ingresa por cuadro clínico que inicia el 21/05/19
consistente en dolor en miembros inferiores intenso
asociado a parestesias de mayor intensidad durante
las noches. Refiere que esta clínica se mantiene,
presentando mayor intensidad el 23/05/19, por lo
que consulta a centro de salud donde indican
analgesia (no refiere que medicamento). El
24/05/19 el dolor en miembros inferiores se asocia
a múltiples episodios eméticos de contenido
alimentario, aprox 15 episodios, por lo que
reconsulta, indican antiemético, con control
sintomático y dan egreso. Familiar refiere que el día
25 de mayo, la sintomatología se agudiza con
imposibilidad para la marcha, con debilidad
 ENFERMEDAD
5 ACTUAL

A la valoración clínica, se encuentran signos vitales normales,

deterioro del estado de consciencia, encefalopatía,
compromiso de pares craneales y componente medular con
síndrome de motoneurona inferior, por lo se decide asegurar
vía aérea mediante intubación orotraqueal. Revisan TAC de
cráneo simple sin evidencia de patología aguda intracraneal.
Se realiza punción lumbar con citoquímico inflamatorio, sin
celularidad, con ADA >9 por la cual se indica cobertura
antifimica y antiviral, sin descartar que pudiera estar en
relación a origen viral vs TBC por antecedentes y hallazgos
lesiones de probable origen herpético.
