DEFINITION
Steroids are naturally occurring chemicals,
produced mainly by the adrenal glands in the
abdomen. There are different types of steroids,
and the type most commonly used to treat skin
problems is known as a corticosteroid.
Corticosteroids treat skin conditions by
suppressing inflammation and reducing the
amount of collagen in the skin. Corticosteroids
that are applied to the skin in the form of
creams, ointments, lotions, mousses, shampoos
gels or tapes are known as topical corticosteroids
(TCs). [1]
HISTORY

Topical corticosteroids (TCs) are of great


value in treating a wide spectrum of
dermatological diseases and since the time
of its introduction in 1951

In 1952, Sulzberger and Witten first treated


eczematous dermatitis with topical
hydrocortisone.
BASIC STRUCTURE
MODE OF ACTION

Corticosteroids have anti-inflammatory,


immunosuppressive, anti-proliferative
and vasoconstrictor actions. Many of
these actions are mediated by the nuclear
glucocorticoid receptor which modulates
transcription of proteins.

In the skin, glucocorticoid receptors have


been located in keratinocytes and fibroblasts
within the epidermis and dermis .
CLASSIFICATION
The WHO classification divides the topical
corticosteroids into seven classes/groups, with
group I being the most potent and group VII
being the least potent.

PHARMACOKINETICS
•Distribution: After application, distributed
throughout local skin; drug absorbed
systemically is rapidly removed from
circulation and distributed to muscle, liver, skin,
kidneys, intestine.[5]
•Metabolism: Mainly in the skin by:
A) Oxidation
B) Conjugation. [5]
•Excretion: Excretion is mainly done by skin
pores and kidneys.[5]
•DETERMINING REQUIRED AMOUNT

Based on Finger tip units

One fingertip (FTU) is the amount of topical steroid


that is squeezed out from a standard tube with a
5mm nozzle from the very end of the finger to the
distal finger crease.
