CCS RETAINER ORIENTATION

A. General Healthway Policies, Rules and Procedures


1. HMC Profile

- Retainer physician must strictly observe and follow the Doctors’ Code of
Ethics under PMA and PCOM.
• Duty Attendance
- Retainer physician must maintain the highest degree of professionalism and
punctuality in rendering or making available his/her medical services at the
clinic.
• Application of Leaves
- Planned leaves – CCS officer must be notified five (5) working days prior the
retainer schedule; and shall have referred reliever/s.
- Unscheduled or emergency leaves – CCS officer must be notified as early as
possible.
3. Credentials
- Retainer physician must maintain at all times a valid PRC license/PTR and OR
and if with specialty, all the required documents to enable him/her practice
the profession including his/her specialization.
- Retainer physician must undertake his/her Occupational Health Training.
B. Contract Discussion
1. Type of Contract Agreement
- The retainer physician contract is an independent contractor agreement. It serves
as a co-terminus with HMC’s term of service to the client.
2. Duration of Contract
- The contract term of the HMC contract shall be effective from April 7, 2019 to May
30, 2019, unless terminated as provided herein.
3. Scope of Services
- Provide daily out-patient consultation
- Provide efficient first-aid and medical intervention to any worksite emergency case
- Organize, administer, and maintain an occupational health service program
integrating therein an occupational safety program in accordance with law and
other appropriate regulatory agencies.
- Evaluate and interpret any medical examination result and give corresponding
recommendations if needed.
- Develop adequate healthcare programs for the employees of the principal.
- Give health teaching, awareness, counseling and education on how to detect and
prevent medical illness and conditions
- Provide regular reports on the illnesses incidence and prevalence and health
status of the Principal.
- Give recommendations to health-related problems affecting the employees of the
Principal/Client Corporation.
4. Complete disclosure of Clinics, Hospitals and Corporate Clinics
- Retainer physician is entitled to engage in other medical practice or employment,
separate and apart from the present accreditation or engagement with HMC,
provided only that the separate practice or employment will not interfere or
prejudice the performance of contractual obligations to HMC under the contract
agreement.
5. Service Hours
- Retainer physician shall render medical services in accordance to the schedule
stated in his/her contract.
- Breaks and/or lunch time are not included.
- More than 30 days of absences, contract may be terminated by HMC, subject to
the approval or consent of the Corporate Client who may agree to temporarily
accept the services of a reliever physician.
- Extension of clinic hours or change of schedules shall be upon the request of HMC
and/or Corporate Clients authorized officer and must be properly documented.
C. Payment Information
1. Retainer Physician Fee
- Retainer Fee will be remitted based on the schedule stated on the contract.
The said fee is subject to the applicable tax rates by prescribed law.
- Retainer physician shall issue an Official Receipt corresponding to the total
monthly remittance.
2. Schedule of Release Payment
- 20th of the previous month to 4th of the current month = 15th of the current
month
- 5th of the current month to 19th of the current month = Last day of the
current month
3. ATM Requirement
- Existing BPI/Metrobank Savings
- Filled in BPI Form with attachment of Billing Statement
D. Endorsement to CCS

1. Corporate Account
- IQOR SM Clark

2. Location of Account
- IQOR SM Clark – Clark SM, 2nd and 3rd Floor Tech Hub 1 & 2, SM City
Clark, MA Roxas Highway, Brgy. Malabanias, Angeles City

3. Officer in Charge
- Retainer physician must coordinate directly to the officer in charge for any
clinic operation, co-medical staff and client concerns.
