Definicion

Déficit transitorio en la perfusión del

flujo sanguíneo coronario, lo que
ocasiona una despolarización parcial
de las células miocárdicas y alarga la
duración del potencial de acción.

Electrocardiograma

El patrón de isquemia miocárdica en el ECG es

una alteración de la repolarización en la que se
ocasiona el descenso del segmento ST y la
negativización de la onda T.

INSTRUCTIONS FOR USE

• Para fines electrofisiológicos el corazón se divide en

subendocardio y subepicárdio.
• Las fibras de Purkinje se localizan en subendocardio
• Despolarización ocurre de endo a epi y la repolarización
es inversa.
• Ondas T normales son positivas.

Tipos de isquemia

Isquemia subendocárdica
◦ Dirección de despolarización y repolarización no cambia.
◦ Demora la repolarización: Ondas T mas altas y simétricas.
Isquemia transmural, subepicardica:
◦ Repolarización invierte su orden y demora mas:
T invertidas y anchas

Descenso del segmento ST

Sugestivo de isquemia sí:

◦ Si es de nueva aparición.
◦ Si es horizontal o descendente.
◦ Si dura al menos 80 ms (2 cuadraditos) y desciende al menos 0,5 mm en el
punto J o a 60 ms del mismo en 2 derivaciones contiguas.

 Tratamiento de cardiopatía isquémica
Tratamiento optimo
El tratamiento óptimo puede definirse como el
tratamiento que satisfactoriamente
controla los síntomas y previene los eventos
cardíacos asociados con CCS,
con adherencia máxima del paciente y eventos
adversos mínimos.

El principal objetivo es reducir los síntomas

Se consigue una reducción absoluta de la incidencia de IM o muerte por causa
cardiaca
El tratamiento medico intenta mejorar la oxigenación del miocardio, reducir
el consumo miocárdico de oxigeno, controlar los factores agravantes (anemia)
y limitar el desarrollo de enfermedad ateroesclerótica

reduzcan los criterios de
valoración de morbilidad
o mortalidad en pacientes
con CCS

 Nitratos
◦ Tanto los preparados de acción prolongada para uso crónico como
los compuestos sublinguales/tópicos para síntomas anginosos
agudos, se usan con mas frecuencia como antianginosos
complementarios.

Las presentaciones de
nitroglicerina sublingual y spray
proporcionan un efecto inmediato
alivio de la angina de esfuerzo.

(0.3-0.6 mg comprimido por vía sublingual y no

ingerido, o 0.4 mg aerosol a la lengua y no tragado o
inhalado) cada 5 minutos hasta que el dolor
desaparece
