ANEURISMA
Dilatacin permanente y localizada de una arteria que tiene, al menos, 50% de aumento en el dimetro, comparado con el dimetro normal de la arteria en cuestin (Sociedad Internacional de Ciruga Vascular) (Journal of Vascular Surgery 1991;13: 444-450)
Antecedentes histricos.
1552 A.C. En Egipto, en el papiro de Eberts se describen los aneurismas post traumticos. Galeno describe en el siglo II un aneurisma arterial post traumtico. Ambrosio Par en el siglo XVI, asoci sfilis con aneurisma Ao. 1757. Willian Hunter describi un aneurisma torcico. Gross en mayo de 1948 realiza la primera reseccin y reemplazo de la aorta. La primera reseccin de la Ao ascendente se reporta en 1956 por Cooley y Debakey. En 1963, Starr reporta el reemplazo de la aorta ascendente con injerto sinttico + reemplazo de la vlvula artica con prtesis. 1968, Bentall y Bonno reportan el reemplazo total de la raz artica y ascendente con tubo sinttico con prtesis mecnica (tcnica de inclusin)

La incidencia de aneurisma de aorta se estima en 5.9 nuevos casos/ 100,000 personas ao. La edad de presentacin es : Hombres : 65 aos Mujeres : 77 aos
FISIOPATOLOGIA
La Ao Ascendente funciona como reservorio elstico, guardandoenerga durante la sstole y disipndolo durante la distole. La Ao ascendente es ms rica en fibras elsticas que la Ao descendente. La mayoria de aneurismas de Ao Ascendentes se asocian a cambios degenerativos de la capa media elstica, mientras que en Ao descendente se asocian ms con aterosclerosis. La combinacin de aneurisma de Ao ascendente + dilatacin de los senos de Valsalva y del anillo se conoce conoce como ectasia anulo artica.
