B) Requirements to be paticipants jamkesda : - Permanent resindents who have KTP. - Work in the pormal sector . - Nor to follow the health insurance program /health benefits otherwise, from governments,private institutions or private health insurance. -Registered in the family card.
C) Registration Reguirements 1.Obtain certificate of RT, head man to become participants jamkesda. 2.The latter was then taken to registered the neigh bothood health center by attaching : - 1 copy of the permanent identity card. - 1 copy of family card. - 2 species of 2x3 photo

D) Hospital entry requirements : 1.Bringing a reference letter from a health center . 2.In case of emergency, may direct to the ER and take reference letter following on from the health center at least 2x24 hours. 3.Once every 10 days to extend hospitalition by taking the letter of guarantee from the secretariat jamkesda. E) Type Of Service 1.Medical consultation and hospitalizition basic healyh education class III physical examination grade III in patien basis. 2. Accemmodation in patien and cating in class III at the hopital ,etc.
