Les tudes comparant les rsultats fonctionnels aprs reconstruction du ligament crois
antrieur par autogreffe au tendon rotulien versus ischio-jambiers sont nombreuses mais trs
peu dentre elles sintressent la morbidit de la phase prcoce de rducation. De mme, la
morbidit prcoce de la tnodse externe au fascia-lata lorsquelle est associe la
reconstruction du LCA nest pas bien documente.

Cette revue rtrospective concerne 209 patients. La rpartition est la suivante : 146
ligamentoplasties aux ischio-jambiers dont 70 avec une tnodse externe associe, et 63
ligamentoplasties au tendon rotulien dont 23 avec une tnodse externe associe. Tous ont
suivi un protocole de rducation adapt au type de ligamentoplastie utilis.

En comparant les greffes utilises pour la plastie du LCA, les patients du groupe ischio-
jambiers montrent un profil douloureux moins marqu que ceux du groupe tendon rotulien
mais sans diffrence concernant la rcupration fonctionnelle globale sur les 6 premires
semaines. Lassociation dune tnodse externe au fascia-lata la plastie du LCA ralentit le
gain de flexion compte tenu des consquences trophiques et algiques lies ce geste extra-
articulaire.
Summary
Studies comparing outcome after anterior cruciate ligament reconstruction using patellar
tendon versus hamstring graft are numerous but very few focus on the early rehabilitation
period morbidity. In the same way, the early morbidity of iliotibial band tenodesis when it is
associated with ACL reconstruction is not well documented.

This retrospective review included 209 patients. They have all followed an adapted
rehabilitation protocol in our department: 76 underwent hamstring graft reconstruction, 40
patellar tendon reconstruction, 70 hamstring graft associated to a iliotibial band tenodesis and
23 patellar tendon reconstruction associated to a iliotibial band tenodesis.

Comparing the different grafts used for ACL recontruction, patients in hamstring group
showed better pain profile than patellar tendon group but showed no difference regarding
global functional results within 6 weeks.

Combination of a iliotibial band tenodesis reduces the gain in flexion, as a result of trophic
and algic consequences related to the extra auricular plasty.
