1. Libur di tanggal 1 januari shift 1.
2. Hujan di tanggal 3, 4, 8, 11, 14, 16, 19, 21, 27 dan 28 januari sehingga mengakibatkan kurang efektifnya kagiatan penambangan dan hauling ore ke efo.
3. Masalah sosial, pemalangan jalan haulling di tanggal 6 Januari di shift 2 (malam) sampai tanggal 7 Januari pukul 21.30 wita.
4. Pemalangan di tanggal 14 januari akibat retaknya jembatan yang disenggol oleh truck tronton.
5. Pemalangan pada tanggal 20 Januari 2022 shift siang sampai malam di depan pos 5.
6. Maintenence jalan hauling yang belum maksimal, khususnya ketika waktu setelah hujan (slippery) sering terjadi keterlambatan scrapping jalan dari tumpukkan lumpur
sehingga sering menghambat waktu dimulainya kegiatan hauling. Penambahan 1 unit grader dapat membantu percepatan scrapping jalan khususnya jalan hauling di area pit.
7. Masalah lahan area Pit 3E yang belum diselesaikan sampai tanggal 20 januari 2022 sehingga mengganggu kelancaran penambangan. Cummulative Retase Ore
8. Fuel Truck-02 mengalami masalah (breakdown) di tanggal 19 Januari 2022 sehingga terjadi keterlambatan pengisian fuel di area efo dan area jetty. Sebaiknya unit yang
jumlahnya minim harus dilakukan pengecekkan unit secara intensif dan diprioritaskan. 12,000
9. Fuel Truck-02 mengalami masalah (breakdown) pada pukul 15.00 tanggal 29 Januari 2022 sampai pukul 17.00 tanggal 30 januari 2022 sehingga terjadi keterlambatan pengisian
fuel di area pit dan efo dan mengakibatkan beberpa unit exca stand by selama 1 shift. Sebaiknya unit suport yang jumlahnya minim harus dilakukan pengecekkan unit secara
intensif dan diprioritaskan.
10. Kondisi jalan haulling yang belum dimaintenence oleh pihak MHR hingga 7 januari 2022.
11. Beberapa unit exca yang selalu stand by karena no operator.
12. Ranting pohon yang sudah menjalar ke area jalan hauling di sepanjang jalan dari pos 2 sampai efo 4 sehingga mengganggu jarak pandang driver. Hal ini sudah ditangani oleh 10,000
pihak MHR namun belum sepenuhnya terselesaikan.
13. BD-02 dan BD-03 kurang efektif saat melakukan kegitan dozing OB di pit, serta seringkali mengalami breakdown.
