1. Cadre Général du projet

L’échographie est une méthode d’imagerie médicale non invasive, facile à mettre en œuvre,
accessible au lit du patient, peu coûteuse, et très informative dans de multiples pathologies.
Cette méthode d’imagerie est particulièrement intéressante dans les pays en voie de
développement et sa promotion a fait l’objet d’un soutien de la part de l’OMS.

Au Yémen, la pratique de l’échographie est caractérisée par une diffusion assez importante de
la technique et par une hétérogénéité importante des pratiques. A coté de grand centres privés
ou publics dotés de matériel et de médecins performants, il existe de très nombreux examens
échographiques produits par des médecins équipés mais dépourvus de formation spécifique.
Compte tenu de son impact sur la santé, la formation continue des médecins yéménite dans le
domaine de l’échographie apparaît essentielle dans le cadre d’une amélioration de la prise en
charge des malades. Suivant les recommandations de l’OMS, une formation progressive par
niveau, répétée, est le seul gage de prestations de qualité. La récente création de la Société
d’échographie yéménite va dans ce sens, et son action formatrice doit être soutenue.

2. L’origine et les objectifs du projet

Ce projet a comme objectif d’améliorer et d’uniformiser les connaissances théoriques et

pratiques en échographie des praticiens exerçant cette technique au Yémen, en intègrant
la reflexion sur la place de cette procédure diagnostique dans l'ensemble de la prise en charge
du patient.
Ce projet est dans la continuité des actions de formation en radiologie, échographie, et
imagerie médicale réalisées par la Mission Médicale Française de Taiz depuis les années
1980, puis de l’Association Médicale Franco Yéménite depuis sa création en 1992. Il entre
parfaitement dans le cadre du projet global cadrant les actions de l’AMFY rédigé en 1999.

Il vient en complément et en soutien des activités de la nouvelle société yéménite

d’échographie (YEMSU), créée en 2002 avec l’aide de l’AMFY, sous l’égide des sociétés
savantes internationales d’échographie, Mediteranean and Asian Society of Ultrasound
(MASU), European Society of Echography (ESE), et World Federation for Ultrasound in
Medicine and Biology (WFUMB).

3. L’organisation du projet et les moyens à mettre en oeuvre

L’organisation pratique de ce cycle continu d’enseignement post universitaire est détaillé ci-
après (document rédigé par le coordinateur du projet, le Pr Michel Claudon).
Le principe retenu prend modèle de ce qui a déjà été testé avec succès en 2000 et 2002
(missions des Dr Claudon, Lefèvre, Hassan Gharbi, compte rendus en annexe).
Il s’agit de deux cycles de conférences par an, d’une durée d’une semaine, réalisées par 4 à 6
experts internationaux. L’audience est composée de 50 à 100 médecins yéménites exerçant
l’échographie, soit en exercice privé, soit en exercice public, venant des différents
gouvernorats du Yémen, et/ou de membres de la YEMSU. Les conférences ont lieu à Sanaa, à
l’hôpital al Thawra, sous forme de présentations théoriques et d’ateliers de présentation de cas
en temps réel, avec vidéotransmission à partir d’un échographe présent dans la salle.

Le programme scientifique et les thèmes à aborder sont précisés dans un document rédigé par
le président de la YEMSU et coauteur du projet,le Dr Adel Mahyoub, en annexe 1

Le montant prévisionnel de ce projet est de 28 620 euros /an (détail du budget prévisionnel en
annexe 2).

L’annexe 3 comporte les compte-rendus des deux dernières mission d’enseignement en

radiologie-échographie organisées par l’AMFY, qui illustrent la parfaite faisabilité du présent
projet et le grand intérêt qu’ont montré les médecins et autorités yéménites à sa réalisation.

Morning sessions :
- conferences presented by the faculty members, and/or Yemeni experts. Each day
one different thema will be chosen in a progressive approach to a large field of
imaging investigation. The choice of the main field , the sub-themas and the
experts will be made in agreement with the Yemeni physicians groups and
Societies ( including Dr Adel Mahyub, Chief of the Radiology Department, and
the new Yemeni Society for Ultrasound)
- discussion of selected cases with the audience (prepared by the experts, and by the
Yemeni doctors)

Afternoon sessions :
- workshops : live demonstration of ultrasound cases from hospitals, using different
ultrasound units. These units will be connected to a videoprojector, as recently and
succesfully done, to allow technique and practice teaching in an interactive way,
with a free discussion between the audience and experts. Other workshops could
be organized, i.e. therapeutic demonstration in coronary / angiography room,
interventionnal procedures demonstration in CT scan room…

Scientific program of Yemsu (Yemen Society of Ultrasound)

General considerations

The activity of the Yemsu will be organized for the next three years based on scientific meetings held twice a year:

- Months of April/may and Septembre/October

- Al Thoarah Teaching Hospital, Sanaa
- sessions of 2 to 3 days
- courses and practical teaching with US machines (normal and patients)
- Invited speakers
o 2 to 4, depending on the scientific program (lectures), and the number of workshops or practical courses
(demonstration with volunteers and mostly patients on ultrasound machines, using interactive teaching)

Contribution of the French Embassy to be defined, in collaboration with the French-Yemeni Medical Association.
Recognition as an Excellence Centre by the WFUMB (World Federation of Ultrasound in Medicine and Biology) to be
applied.
