Résumé
Le Staphylococcus aureus résistant à la méticilline (SARM) pose un problème de traitement surtout avec l’émergence de souches de
sensibilité diminuée voir résistantes aux glycopeptides (GISA). Le but de notre travail consistait à évaluer la résistance des SARM aux
antibiotiques, et de déterminer la fréquence et la caractérisation des souches de GISA. L’étude de la sensibilité aux antibiotiques a été réalisée
pour 620 S. aureus. La détermination de la CMI de l’oxacilline, de la vancomycine, et de la teicoplanine n’a concerné que les 96 (15,5 %)
souches de SARM. L’analyse globale de la sensibilité aux antibiotiques a confirmé le caractère multirésistant des SARM mais avec une plus
fréquente sensibilité à la gentamicine. L’étude de la sensibilité aux glycopeptides a permis de détecter deux souches (M4 et M41) de sensibilité
diminuée. Le gène mecA n’a été détecté que dans la souche M4 qui héberge un « Staphylococcal Cassette Chromosome » mec (SCCmec) de
type III, alors que le locus de la leucocidine de Panton-Valentine (PVL) a été mis en évidence dans les deux souches. Il s’agit de la première
description de souches de GISA en Tunisie. De telles souches semblent être rares mais une surveillance régulière de la résistance des SARM
aux antibiotiques est nécessaire.

Abstract
The adaptation of Staphylococcus aureus to the hospital environment led to the acquisition of resistance to all antibiotics available in
clinical practice. The aim of this study was to investigate the methicillin-resistant Staphylococcus aureus (MRSA) strains isolated in the F.
Bourguiba hospital of Monastir (Tunisia). We determined the antibiotype of all Staphylococcus aureus strains identified. Susceptibility rates
to fosfomycin, chloramphenicol, rifampicin and pristinamycin were 7%, 3%, 2% and 0%, respectively. The prevalence of MRSA was 15.5%
(96 strains); their susceptibility to gentamicin progressively increased. The minimum inhibitory concentration (MICs) of oxacillin, vancomy-
cin and teicoplanin were evaluated for the 96 MRSA strains. We identified two MRSA strains (M4 and M41) showing reduced glycopeptides
susceptibility. Further analysis revealed that M4 and M41 harbor the gene encoding the class S and class F proteins specific for the Panton-
Valentine Leukocidin (PVL). The mecA gene was detected only in strain M41 which harbors the Staphylococcal Cassette Chromosome
(SCCmec) type III. This is the first reported MRSA showing reduced susceptibility to glycopeptides in Tunisia. Regulatory surveillance of
susceptibility to antibiotics is needed to reduce the morbidity and the mortality rates as well as societal costs of S. aureus infections.
