3. THROMBOSE, THROMBOPHILIE
MESSAGES CLEFS
• La grossesse induit une hypercoagulabilité due à l’augmentation des facteurs
de coagulation, à la baisse de la protéine S, anticoagulant naturel et à l’inhi-
bition de la fibrinolyse par le PAI-2.
• La prééclampsie s’accompagne d’un syndrome inflammatoire systémique
maternel et d’un excès de génération de thrombine associée à une thrombo-
pénie et à un risque thrombotique.
• L’hémorragie du post-partum sévère et les grandes catastrophes obstétricales
comme l’embolie amniotique et l’hématome rétro placentaire s’accompagnent
d’une coagulopathie active et intense. Celle-ci peut être détectée précocement
par des examens délocalisés de l’hémostase.
• La fibrinogénolyse et la fibrinolyse, au centre du processus, peuvent être
corrigées par les concentrés de fibrinogène et l’acide tranéxamique ainsi que
la transfusion de plasma.
