QUELQUES DOSES PEDIATRIQUES USUELLES POUR STAGIARE EN MEDECINE

I. Electrolytes, vitamines et liquides

- Na+ : NaCl amp 1,71mEq/ml, amp 10ml : 1-2mEq/Kg/j
- K+ : KCl amp 1,34mEq/ml, amp 10ml : 1mEq/Kg/j
- Ca2+ : Gluconate de calcium : 2cc/kg/j chez le NNé et 0,2-0,5cc/Kg chez le nourrisson
et l’enfant en IVL
- MgSO4 : 0,5cc/Kg/j
- Vit B complexe : 0,5cc/kg/j
- Konakion amp 10mg/2ml : 1-2mg/kg IVL, IM
- SG10% 60-80ml/Kg/j au J0 chez le NNé
II. Vermifuges, Antiparasitaires, anti amibiase, et anti diarrhéique
- Vermox (Mebendazole) : co 100mg, sp 100mg/5ml : 2 x 100mg/j//3j
- Albendazole (Zentel, Zeben, Helmintox, …): co 400mg, sp 400mg/10ml: 15mg/Kg en
prise unique
- Metronidazole (menogyl, Flagyl,…) : co 250mg, co 500mg, sp 125mg/5ml, fl
500mg/100ml : 30mg/kg/j en 3 prises
- Meyamicine : co 500mg , sp : nrsson 2 x1cac/j, enft 3 x 1cac/j
- Zinc (Cezinc, Zinque) : co 20mg, sp 20mg/5ml : inf 6mois 10mg/j, sup 6mois 20mg/j
- Bifidobacillus (Ultralevres, floranorm, bacilor,…) : 1-3sachet/j en 2 ou 3 prises

III. Anti grippal, anti histaminiques

- Niférhinol : sp, co : 6mois-2ans 3 x 1cac/j ; 2ans-5ans 3 x 2cac/j ; 5-12ans 3 x 1cas/j
- Argyrol gtte : 0,5% (nrson) et 1% (enft sup 3ans) : 3 x 2-2gtte/narines/j
- Solution physiologique gtte : 3-4-6 x 2gttes/narines/j
- Prométhazine: sp 5mg/5ml, amp 25mg/1ml: 0,5mg/kg en 2 prises
- Phenergan (Cetrimex, …) : co 10mg : 0,5-1mg/kg

IV. Anti inflammatoire, antipyrétique, antalgique et anti émetique

- Ibuprofène (Pedifen, brufen, brumed, anaflam,…) : co 400mg, sp 100mg/5ml : 5-
10mg/kg/prise avec un maximum de 3 prises
- Kétoprofène (Profenid) : sp 1mg/ml, co 50mg, co 100mg, amp 100mg/2ml : 0,5-
2mg/kg/j en 3-4 prises sans dépasser 300mg/j
- Nifluril (ac niflumique, morniflumate) : suppo 400mg : 6-30mois 200mg/j en 2 prise,
30mois-12ans 1suppo par 10kg de poids/j (max 3 suppo)
- AAS … : antiaggrégant 3-5mg/kg/dose ; antitpyrétique 10mg/kg/dose ; anti
inflammatoire 20-25mg/kg/dose
- Anset (ondasetron) amp 2mg/ml, co 4mg, suppo 4mg: 0,2mg/kg par prise
- Motilium sp 200ml : 1mg/kg en 3 prises
- Debridat sp : 3 x doseKg/j (ou 1ml/Kg en 3 prises)
- Paracetamol (Efferalgan, doliprane, pandol, amidol,…) co 500mg, amp 750mg, suppo
(perdolan) 100, 150, 200, 300mg, sp 125mg/5ml : 15-20mg/kg par prise (max 4 prise
journalière)
- Morphine …. : 1mg/kg/j PO en 6prises ; 0,1-0,2mg/dose IV bolus toutes les 2-4h ;
0,01-0,04mg/k/h en IV continu
V. Anti convulsifs
- Phenobarbtal (Luminal, Gardenal,..) : co 10, 50, 100mg : 20-40mg/kg/j en dose
d’attaque chez le NNé et 10-15mg/kg/j en dose d’entretien (à diminuer
progressivement), 10-15mg/Kg/j chez le Nrson et enft en dose d’attaque et 3-
5mg/kg/j en dose d’entretien
- Diazépam co 10mg, amp 10mg/2ml : 0,3mg/kg par prise en iv dilué et
0,5mg/kg/prise en IR dilué
- Midazolam amp… : 0,05-0,2mg/kg IVD
VI. Corticoïdes et aérosols
- Prednisolone (Cortancyl cotipred,…) : co 5, 20mg : 0,5-0,7mg/kg/j
- Célestène (betamethasone): co 0,5mg, amp 20mg/5ml, sp 0,2%: 0,075-0,3mg/kg/j ou
1 x gtte-kg/j
- Hydrocortisone amp 100mg/5ml: 5mg/Kg par prise. Au plus 3 prise/j
- Dexaméthasone amp… : Méningite 0,15mg/kg/dose toute les 6h ; autre 0,25-
0,5mg/Kg/dose toutes les 12h
- Salbutamol (sln de 0,5% goutte,…): 0,01-0,03ml/kg/aérosol toutes les 20min ; 0,001-
0,01mg/kg/min
VII. Pansement gastrique, IPP, Anti H1
- Maalox (pansement gastrique)susp : 3 x 1cac chez enft inf 5ans ; 3 x 1cas chez enft
sup 5ans
- Oméprazole, Rabeprazole : cps 20mg : 1-2mg/kg/j sans dépasser 20-40mg/j
- Azantac (ranitidine) amp 50mg/2ml : 5mg/Kg/j
VIII. Anti paludiques
- Arthémeter-Luméfantrine (Luther, L-artem, lonart, cofantrine, coartm, corimetar,…) :
co 20/120mg, 40/240mg, 80/480mg ; sp 180mg d’arthémeter/60ml : 4mg/Kg en
fonction de l’arthémeter/j en 2 prises
- Artesunate inj (artesun, start, AT, M-arrêt,…): 3mg/kg pour enft inf 20kg et 2,4mg/kg
enft sup 20kg par prise à Ho-H12-H24
- Quinine sel (sel de chlorydrate=Surquina) : co 300mg, 500mg, sp 100mg/5ml : 30mg/j
en 3 prises dans 5cc/kg de SG5% pdt 4h avec pause de 8h
- Quinine base (Quinimax): co 250mg, 500mg : 24mg/j en 3 prises dans 5cc/kg de
SG5% pdt 4h avec pause de 8h
IX. Antibiotiques
- Amoxicilline (Clamoxyl, Bactox, amoxicilline,…) : sp 250mg/5ml, sp 125mg/5ml, sp
500mg/5ml, cps 500mg, amp 500mg, amp 1g: 30-50mg/Kg/j en 2-3prise peros ; 100-
200mg/kg/j en 3 prises IV
- Amoxicilline-ac clavulanique (Augmentin, curam, dafraclav, aclav, arauclav,…) : sp
100mg/5ml, 125mg/5ml, 250mg/5ml, cps 500mg, 1g : 25-50mg en fonction de
l’amoxi/kg/j en 3 prise p.o et 100mg/kg/j en 3 prises iv
- Pénicilline G : 25000-100000 U/kg/dose IV lent toutes les 6h
- Cefuroxime (Zinnat,…) : sp 125mg/5ml : 20-30mg/kg/j en 2 prises p.o
- Cefixime (Oroken pipette graduée, cefinique, efficef, …) : sp 40mg/5ml, sp
100mg/5ml, co 100mg, co 200mg : 8mg/kg/j en 2 prises p.o
- Cefotaxime (Claforan, alcef, ouicef, cefatx,…): fl 1g: 100-200mg/kg/j iv en 3-4 prises
- Ceftriaxone (C-tri, rocephine, stericef, G-ceftria, cetafor,…): fl inj 1g: 50-100mg/kg/j
en 1-2 prises
- Cefipime inj: 50mg/kg/j iv en 2 prises
- Amikacin : fl 100mg, 500mg : 10-15mg/kg/j en 1 prise
- Gentamycine amp 75mg : 3-5mg/kg/j en 1prise
- Pipéracilline-tazobactam (Piptaz, Car, dozar, …) : fl 2,25g : 90-100mg/kg/prise en 3
prises en fonction de pipéracilline
- Clindamycine (Dalacin, Clinda,…) cps 150mg, 300mg, amp 600mg : 10mg/kg/prise 3
fois par jour
- Vancomycine fl 500mg, 1g : 40-60mg/kg/24h en 4 prises
- Erythromycine : sp 125mg/5ml, co 250mg, 500mg : 10-20mg/kg/j en 3 prise p.o
- Lincosamide (Lincocin): sp 250mg/5ml; amp 300mg, 600mg: 30-60mg/kg/j p.o en 3
prises et 10-20mg/kg/j iv en 2prises
- Fluocloxacilline (Floxapen, Flux, Astaph,…) : sp 125mg, 250mg/5ml, co 500mg : 25-
50mg/kg/j en 3prises
- Bactrim : sp 240mg/5ml, co 480mg, 960mg : 4-8mg/kg/prise en fonction de
sulfaméthoxazole, en 2 prises
- Ciprofloxacine (Cifin, cipronat, …) : fl 200mg/100ml, co 250mg, 500mg : 20mg/kg/j en
2 prises
- Nitrofurantoine (Furadantine) : co 50mg, 100mg : 5-7mg/kg en 2-3 prises
- Azythromycine (Cemycine, Alzith, Zythromax, …) : 15-20mg/kg/j en 1 prise
X. Diurétiques
- Lasix : co 20mg, 20mg, amp 20mg : 1mg/kg
- Hydrochlorothiazide…: 1-1,5mg/kg PO toutes les 12h
XI. Réanimation
- Adrébaline amp …: NNé 0,01-0,03mg/kg/min IVD; Enft 0,01mg/kg/min IVD
- Atropine… : 0,01-0,02mg/kg/dose IVD
- Caféine amp 250mg/… : Charge 20mg/kg puis 5mg/kg IVL
- Dobutamine … : 0,02-0,2mg/kg/min
- Dopamine…. : 0,02-0,2mg/kg/min
XII. Anti hypertenseurs
- Enalapril co 5mg : 0,1mg/kg/dose (max 0,5mg/kg/dose) toutes les 12 voire 24h
- Labétalol… : 1-3mg/kg/heure
- Nifédipine…. : 1-4mg/kg/j po en 4-6prises
