DECLARATION

Upon discovering some error in “Annex C - Questionnaire form”, immediate

action was taken to rectify it and ensure the accuracy of the information
provided. The mistakes was the result of typographical errors and oversight
by the individual responsible who edited the “Annex C”.
The typographical errors apply to paragraphs III.3, V and VII areas and are
replaced by the following areas.

VII. Declaration of consent for the publication of image and video material and transfer of image
rights/Déclaration de consetement pour la publication de matériel d´image et de vidéo et tranfert des
droits d´image

I / we agree that the HNDGS will take photos and record videos of my/our son/daughter in the course of this summer camp and that any of these video recordings
and/or photos may be used for publications on websites and other publications of the HNDGS. The HNDGS is entitled to a timely and locally unlimited and
unrestricted implementation, use and storage of images. For its part the HNDGS assures that the recordings will be used for the purposes of the association only.
Je / nous convenons que le HNDGS prendra des photos et enregistrera des vidéos de mon / notre fils / fille au cours de ce camp d’été et que l’un quelconque de ces
enregistrements vidéo et / ou photos pourra être utilisé pour des publications sur des sites Web et d’autres publications du HNDGS. Le HNDGS a droit à une mise en
œuvre, à l'utilisation et au stockage des images en temps voulu, localement illimités et sans restrictions. Pour sa part, le HNDGS assure que les enregistrements
seront utilisés uniquement aux fins de l'association.
