2. INCIDENCE DU LAST
Les analyses de l’ASRA [1, 15] concernant l’apparition des LAST montrent
clairement une diminution significative des LAST grâce à l’apport de l’échogra-
phie dans les blocs nerveux. Cette diminution est estimée de l’ordre de 65 %,
ramenant le risque de LAST à 2,6/10 000 patients. Toutefois, il est à noter que
l’utilisation de l’échographie n’a pas complètement éliminé le risque de LAST
dans la pratique de l’ALR. De même sa prise en compte doit rester importante
chez tous les cliniciens utilisant des AL [16, 17].
CONCLUSION
La prévention du LAST doit recueillir toutes les attentions : injections frac-
tionnées, aspiration, vision directe constante de la diffusion grâce à l’échographie
mais aussi dose, volume et concentration des AL adaptés au patient, au lieu
d’injection, au type de bloc réalisé.
