Coccidiose intestinale due à Isospora belli qui parasite les cellules
intestinales de l'homme.
Elle est liée au péril fécal et fréquente dans les régions tropicales et
subtropicales surtout chez les personnes immunodéprimées.

Cycle direct et se déroule dans entérocytes
Après l'ingestion oocystes, sporozoïtes libérés dans lumière tube
digestif et pénétrent dans cellules épithéliales
Cycle schizogonique (cycle asexué): trophozoïtes se transforment en
schizontes multinuclés. La cellule-hôte éclate et libère des
mérozoïtes qui gagnent de nouvelles cellules intestinales
Cycle sporogonique (cycle sexué): mérozoïtes se
différentient en microgamète mâle et en macrogamète
femelle
Fécondation qui donne oocyste qui est expulsé de cellule
hôte et libéré dans milieu extérieur
Maturation oocyste
(sporogonie) à
l’extérieur et aboutira
à l’individualisation
successive dans
l’oocyste de 2
sporocystes contenant
chacun 4 sporozoïtes

Coccidiose intestinale due à protozoaire intracellulaire
du genre Cryptosporidium qui parasite les cellules
intestinales de l'homme et des animaux (veau,
chevreaux, porcelet,…..)

plus grande fréquence dans les pays à bas niveau
socio-économique

Cyclospora cayetanensis (Cyclospora) est un agent
infectieux émergent
Il a été récemment identifié en 1993 comme pathogène
pour l’homme
C'est un protozoaire qui envahit le petit intestin de
l'humain surtout immunodéprimé

Après ingestion oocyste mature, sucs digestifs libère
sporozoïtes qui pénètrent dans entérocytes et se
transforment en trophozoïtes; débute la schizogonie
Après rupture entérocytes infectés, il peut y avoir évolution
vers stade sexué, avec formation de gamètes mâles
(microgamétes) ou femelles (macrogamétes); fusion donne
naissance à oocyste immature excrété dans les selles
Dans milieu extérieur, oocyste immature subit maturation
de 2 semaines et devient oocyste sporulée, qui est forme
infestante pour homme

Chez le sujet immunocompétent :
durée incubation courte, en moyenne une semaine, (de
un jour à un mois selon les cas)
Principal signe: diarrhée subaiguë sans signe de gravité,
fréquence comprise entre trois et dix selles par jour
Evolution spontanément résolutive en quelques jours
Parfois troubles digestifs (douleurs abdominales,
flatulence, perte d’appétit, amaigrissement pouvant
atteindre 5 à 10 kg, nausées, vomissements, etc.) et
asthénie progressive, fièvre modérée
Chez le sujet immunodéprimé :
Plus particulièrement PVVIH, cyclosporose devient sévère
et prolongée
Diarrhée chronique de quatre mois voire beaucoup plus
Cachexie, déshydratation très sévère…
Des localisations extra-intestinales (cholangites) ont été
décrites chez des PVVIH
